Our college is planning to offer three new programs such as Bachelor in Human Services,
Bachelor of Public Management and Bachelor of Science in Entrepreneurship to cater the
needs of the community to address the demand of manpower in such field.

In line with this, the undersigned are requesting your good office to furnish the following
documents as the requirements of the Commission on Higher Education (CHED) to make this
plan possible.

Here are as follows:

1. Copy of Transcript of Record, Appointment and Professional License
2. Library Sitting capacity (combined number of students and teachers)
3. Photo of Facility (floor plan)
4. Library Collection (books, journal, magazines, dictionaries, almanac, etc)
a. List of 5 non-duplicated book titles per subject in the curriculum published within
5 years
b. List of book collection/accessioned books
i. Start up 3,000 library collections (for initial permit)
c. List of subscription to relevant professional journals
(Note: list should be in spreadsheet to include author, title of the book, year of
publication and number of volumes)
Please furnish in three (3) copies.
