An Order Reorganizing the Municipal Rabies Control Council of the Municipality of

Santa Rita, Pampanga

Section I. – Municipal Rabies Control Council:

Pursuant to SB Ordinance No. 04 – 2001 dated November 19, 2001, there is

hereby formed/organized Municipal Rabies Control Council in the Municipality of Sta.
Rita, Pampanga to be composed of the following:

Section II. – COMPOSITION:

Hon. Ferdinand L. Salalila - Chairperson

Municipal Mayor

Lutgarda G. Valencia - Vice-Chairperson

Municipal Agriculturist
Hon. Jay R. Cruz - Member
SB Member/Chairman Comm. on Agriculture/
Liga President

Hon. Orlando Santos - Member

SB Member/Chairman Comm. on Health

Dr. Lennard de Vera - Member

Municipal Health Officer

Ly Franni M. Lacanlale - Member

MLGOO

PSI Leonardo M. Lacambra - Member

Chief of Police

NGO Representative

Section III. – Functions:

The Rabies Control Committee shall be responsible in planning implementing,

monitoring and evaluation of rabies prevention and control activities, information and
education, rabies surveillance, vaccination and other activities.

Section IV - MEETINGS. The Council shall meet every 3 months at such date,
time and place or as often as may be necessary at the call of the Chairperson or
a majority of its members.

Section V – EFFECTIVITY:

This order shall take effect immediately.
