1. INTRODUCTION & SCOPE OF WORKS:

1.1 Introduction:
This Health, Safety, and Environment (HSE) Plan is applicable for Genpact Project. Scope of
works is detailed in the Enquiry Documents – Drawings and Specifications.
This HSE Plan is a description of IBSPL HSE policy, the organizational structure and
responsibilities, procedures & plans and the controls implemented and maintained to ensure that
this project is executed safely and in total compliance with the contract HSE requirements, HSE
Policy and the applicable client requirements.
The requirements of this HSE Plan extend to all employees of IBSPL who will work or visit the
site, including any suppliers or subcontractors.

1.2 Scope of Works:

Leak Detection System, Fire Exit Signages, Fire Extinguishers and Fire Curtains

2. MANAGEMENT COMMITMENT & ACTIVE PARTICIPATION
2.1 IBSPL is totally committed to the safe operation of the project in accordance with all contractual
mandatory requirements, local statutes and general good practices.

2.2 We believe that leadership lays the foundation upon which, a solid safety program is built and
reflects management commitment in its implementation. Effective leadership and program
administration are vital to the IBSPL of safety, health and environment - loss control program.

2.3 HSE Policy duly signed by the Managing Director is attached in the subsequent page. In order to
communicate down the workforce to be understood. Periodic letters and memoranda (or any
other suitable medium) will demonstrate the Management support to the policy on safety,
inspection tours, group meetings, and HSE audits as described in individual sections. This Policy
will be displayed in offices and prominent work places at sites and stores as applicable.

2.4 The Management are committed to all HSE initiatives already being implemented by the Project
Managers and will endeavor to contribute its best to the excellent performance record on site.
2.5 HEALTH, SAFETY, AND ENVIRONMENT (HSE) POLICY

Infiniti is committed to providing a safe and healthy workplace and conducting its business activities in a manner
that protects the environment. To achieve these goals, all our projects shall adhere to the following principles:

 Identification and evaluation of all Occupational Health and Safety (OH&S) hazards and the
management of these risks to reduce their impacts to acceptable levels,
 Identification of all Environmental Aspects & Impacts and the management of these to reduce their
impact on the environment,
 Compliance with all applicable HSE legislation and Other Requirements,
 Prevention of incidents, injuries, and ill Health,
 Intolerance of the conditions and behavior that contribute to incidents and injuries,
 Prevention of pollution, reduction of waste and conservation of resources,
 Recording and communicating HSE performance throughout the organization,
 Continual improvement of HSE performance.

The Board of Directors and senior management shall visibly uphold these principles throughout the Company
and integrate them into the Company’s HSE Policy and Management System. The senior management of the
Company shall regularly review HSE performance.

Management and supervisory personnel at each department, project and location shall be responsible for
implementing and maintaining the Company’s HSE Policy and Management System. Similarly all subcontractors
shall work in accordance with the Company’s HSE Policy and comply with all applicable HSE legislation. Regular
HSE audits shall be carried out to determine conformance to the HSE Management System.

Every employee, whose work may create a significant HSE impact, shall be trained and held accountable for
complying with the Company’s HSE Policy and related procedures, practices, instructions and rules. Each
employee has a duty to report workplace conditions or practices that pose a safety hazard or threaten the
environment and to take reasonable actions to alleviate such risks.

3. HSE OBJECTIVES
 IBSPL believes that pursuit of high standards of health & safety and a positive approach towards
protecting environment is an integral part of efficient management in construction industry.

 The ultimate aim of the management is to have accident free project. Our current targets are:

 All employees will be trained and made aware of the HSE requirements of the project and all
mandatory requirements.

 HSE performance will be monitored throughout the project. The statistics will be reviewed and
efforts will be made towards continuous improvement.

 All activities will be carried out using efficient and safe work practices – clear construction plans,
methods of work and safety guidelines.

 Hazard identification and risk assessment will be carried out on a regular basis and necessary
control measures will be implemented. Necessary posters on related hazards will be displayed.

 All accidents and incidents will be investigated and reported to the management and where
applicable, to the relevant government authorities.

 Management would welcome all constructive ideas and suggestions towards improving HSE
performance.

4. Gate Management Procedures

Each new trade contractor employee or Sub-Contractor or Consultant (other than vendors visiting
office only) is required to attend Project HSE induction session before commencing work on site.
The HSSE Officer will conduct the SAFETY INDUCTION training. The Site new contractor
orientation training Briefing shall include but not be limited to the following : -
 Site specific information relating to the project.
 Induction to the contractor’s site team and their roles and other activities on site.
 Site establishment indicating site office, toilets, amenities, first aid and work area locations.
 Emergency and evacuation procedures.
 First aid, accident procedures, emergency reporting procedures.
 Employee’s responsibilities relating to safe working practices.
 Site regulations covering such items as house rules, general behavior,
 exposure to occupied buildings and their occupants,
 Site specific items. I.e. temporary power, lift usage and goods handling.
 Site hazards and need to follow safe work methods.
 Vehicle &Truck Safety, House Keeping, Use of hazardous chemicals PPE, FPP
Programmed (Hot work, Height work, Confined space work, Work with EEW, Etc.), JSA,
Crane safety For the new comers an induction training programme will be arranged at the
site to provide them with basic idea about the project’s HSSE aspects. Safety induction
following topics shall be covered while given Induction Training and Good safety induction
room should be required.

Only after the completion of Safety Induction Training, persons HSSE department will issue ID card
for staff & workers. No one is allowing to work without ID cards. ID card is stamp with ‘INDUCTED’
stamp.
Trade and Skill Training
Appropriate training is given to ensure that a jobholder, either supervisor or worker, is competent to
do his job safely. Trade and skill training that is required in the construction industries are:
Lifting Supervisor Licensed Electrical Workers
Crane operator First Aid Training
Scaffold Supervisors Rescue Team Training
Scaffold Erectors Powered Tools Operator
Height Works Excavation Work
Slinging and Rigging Operator Abrasive Wheel Changing
Confined Space/Entry Practices Drivers of mobile equipments Contractors shall ensure that such
training is provided at periodic intervals. All training information, records, and certificates will be
properly documented and made available for verification.
Tool Box Talk
Every day at least one Tool Box Talk will be conducted for workmen before starting of the job.
Topics and schedule shall be arranged by the HSSE Officer. The site Engineer/ Supervisor will take
active participation.
Visitor Safety
While at work site all personnel including visitors & suppliers shall wear the basic PPE i.e. hardhat
and safety footwear as minimum requirement. Appropriate and approved personal protective
equipment and protective clothing shall be used depending upon the nature of Work performed i.e.
Job specific PPE. Safety induction and ID card data should be sending to site safety representative
daily basis.

5. COMMUNICATION AND MANAGEMENT OF THE WORK

5.1 Project Organization Structure


Responsibility and Authority are the basic management concepts. Management provides employees
with the authority to carry out assigned tasks and holds the employee accountable for IBSPL successful
and safe completion of the specific tasks. All Managers, Engineers, Staff and Employees shall be
accountable to meet the IBSPL Safety goals and objectives.

5.2 Health and Safety Goals


5.2.1 Full incorporation of HSE Management System into Project HSE Plan.
5.2.2 To fully comply with the requirements of the IBSPL safety policies and procedures.
5.2.3 Develop positive Health and Safety Culture throughout the project.
5.3 Arrangements for monitoring Health and Safety Performance
HSE implementation plan and the inspection schedules of various staff to monitor the safety
performance of the project to meet legal, HSEMS etc. will be advised by the Project Manager.
All shortfalls will be identified and appropriate corrective and preventive action shall be taken within
appropriate time-scale.

5.3.1 Routine Inspections:


Project Manager will carry a routine inspection along with the site HSE representative every workplace at
least once a week and the “HSE Observation Report” will be used to record the observations made then
the remedial actions to be taken. Subsequent remedial actions will be implemented and closed out
through an agenda item at the weekly production meetings.

5.3.2 Workplace Inspections:


Personnel nominated by the Project Manager will carry out the necessary statutory inspections such as
plant, store and lifting equipment, fire precautions and electrical items on regular basis.

5.3.3 Safety Personnel Monitoring:


The Site Safety Officer and Project Manager will monitor the premises on Daily, Weekly and Monthly
basis and produce a written report on the HSE Observations report form. Area to be inspected are
Project Site (General), Welfare Facilities and Office Area.

5.3.4 Directors Review:


The Directors will coordinate with project manage routine basis and maintain the record. Issues raised
will be reviewed again through the weekly production meeting and the solutions will be communicated to
the Director for closed out.

5.3.5 Health and Safety Inspection and Audits:


Inspection on PPE, Harness and lanyard, ladders, hand tools, fire extinguishers, lifting accessories,
machinery and equipment etc. will be carried out by the nominated personnel from the site.
Auditing will be carried out by internal team or external team at the request of Project Manager. Internal
HSE Audit will be carried out by site HSE representatives every month and record will be maintained in
Monthly HSE Audit form.

5.4 Arrangement for Communication:


HSE implementation Plan shows the health and safety meeting arrangements to identify any weakness
in the system and to take corrective or preventive actions accordingly.

5.4.1 HSE Induction Training:


5.4.1.1 Health and Safety Induction by Safety Officer to all employees including visitors.
5.4.1.2 Project specific induction shall be conducted before performing any work.
5.4.1.3 Attendant names and sign will be recorded in the Form and induction stickers will be given.
5.4.1.4 Project Administration department and head of departments will inform Safety Department as soon as
the new comers, visitors, persons arrived from different sections.

5.4.2 With the Production Team:

5.4.2.1 Project Health and Safety Meeting will be conducted on WEEKLY basis and minutes of meeting will be
recorded as per Form this meeting will be chaired by the PM and agenda/minutes of the meetings will be
created by Safety Officer.
5.4.2.2 Weekly Progress Meetings including Health and Safety as a top agenda.
5.4.2.3 Daily Tool Box Talk meetings will be conducted by the respective foreman and engineer.
5.4.2.4 Formal correspondence emails, memorandums etc.
5.4.2.5 Safety Alerts and Information.

5.4.5 IBSPL Mandatory Standard:


IBSPL Policy Statement is attached in this Plan. Copy of the same will be displayed at prominent
locations.

5.4.7 Supervision Standards:


5.4.7.1 Standards of Supervision levels on projects will be determined according to the level of risk involved in
the specific tasks and the competence of those undertaking the task.
5.4.7.2 IBSPL's Project Manager and senior Engineer will determine the levels of supervision required by the
Site Personnel.
5.4.7.3 For Subcontract work, the Project Manager and the subcontractor will agree supervision levels prior to
award of subcontract and these will be recorded.

5.5 Responsibilities towards HSE

5.5.1 Management is responsible for the overall Health, Safety, and Environment Management. This
responsibility is delegated to the Project Manager who will ensure Company and Contract HSE
requirements are implemented on the site with the assistance of Site HSE representative and other
members of his site team.

5.5.2 The responsibilities of the Project Management team on site towards safety are detailed below:-

Project Director

 The Project Director must ensure that adequate arrangements are in place to safeguard
the health, safety and welfare of all persons at site.
 Lead by example.
 Monitor the implementation of this HSE plan at regular intervals in close coordination
with the Project Manager and the project HSE representative.
 Ensure that necessary resources are provided for the proper implementation of the
safety requirements of the project.
 Ensure compliance with all the applicable HSE legislation.

Project Manager

 Ultimately responsible for overall HSE implementation at project site.


 Review and submit HSE Plan for approval by the Client.
 Implement approved HSE Plan in conjunction with Company HSE Policy and Procedures and
those of Client.
 Ensure that works starts only after getting required NOC and approval
 Ensure subcontractors/Supplier full compliance with Safety Rules and Regulations
 Ensure that safety observations and comments are immediately attended
 Ensure, accidents if any occurred are immediately reported to the relevant authorities as per
approved system
 Ensure full participation in HSE Promotion and Performance monitoring.

Project Engineer

 Engage suitably competent and adequately resourced staff and ensure that the Project
targets are met.

 Comply with all rules and regulations listed within the Tender and Contract Documents
and the ‘Project HSE Plan.

 Ensure these rules and regulations are made known to all employees and visitors at
safety inductions, and that these rules and regulations are displayed where all may see
them. Monitor the safety performance of the site on a day-to-day basis in line with the
requirements of this safety plan and any other requirements issued from the clients from time
to time.

 Conduct/attend weekly safety meetings on site to resolve any site safety issues.

 Ensure regular Management Safety briefings to all the staff.

 Ensure the competency required for all his employees.

 Ensure that all staff refers to Risk Assessment to understand hazards and comply with
control measures.

 Carry out Risk Assessment with the team consisting of all concerned persons for all
critical/new activities for which no risk assessment has been carried out.

 Ensure that the concerned site engineers attend to the HSE Officer/ Engineer’s
recommendation via his daily observation report.

 Responsible for the total compliance of this HSE plan.

 Providing adequate manpower for carrying out housekeeping on a regular basis and in
addition fire watch wherever required.

 Ensuring pre job safety meetings with sub-contractor are conducted before they start
their job.

 The direct supervision and responsibility for significant or hazardous high risk works will be
under the close control of each Subcontractor, and this will be monitored by the Safety
team.

Site Engineer

 Ensure all related works are carried out in compliance with approved safety procedures and
applicable regulations
 Ensure work commencement only after receipt of relevant permit/clearance.
 Ensure 100% supervision of all work activities
 Ensure that all environmental protection activities are carried out at site
 Carries out prompt investigation and reporting of all accidents and incidents occurring under his
sphere of control and ensures all recommendations are implemented
 Extend full support for Company / HSE audits
 Ensure that emergency evacuation drills are conducted at least once every 3 months.
 Ensure foremen and supervisors along with the Safety Officer conduct daily tool box meetings
and that all his workmen and supervisors attend the same
 Contribute to safety committee meetings as required by Project Manager
 Implement Near Miss incident reporting and Accident Reporting
 Initiate disciplinary action against offenders/violators of company safety procedures.
Supervisor / Foremen and Charge hands

 Implement the Company HSE rules and regulations for works done under their area of control
 Ensure works being done have a valid work permit signed by Area Authority and that all
conditions stated therein have been implemented and understood by the workforce.
 Advise Project Engineer of training needs of his workforce
 To be alert to all hazards and potential hazards and ensure these are eliminated
 Conduct daily tool box meetings along with the Safety Officer and record of all toolbox meetings
will be kept available.

Project HSE Engineer /Safety Officer

 Assisting the Project Manager in implementing the guidelines given in this document including
review of risk assessment.
 Inspect the work site daily to report and correct unsafe methods and conditions. Making on-the-
spot corrections of unsafe acts of the workmen and taking suitable steps to eliminate all the
unsafe conditions. He will give an inspection report to the Project Manager every day and a copy
to head office HSEM.
 Organizing the inspection of ladders, power tools and machinery on a regular basis.
 Keep a permanent record of all injuries, fires, property damage and crane/ heavy equipment
accidents, which have occurred at the site.
 Conducting monthly safety meeting and keeping record of safety meeting on site complete with
subject discussed and a list of attendees.
 Ensure adequate fire and safety equipment and PPEs are available at the work site and used by
workforce.
 Ensure all operatives have undergone site safety induction and are aware of all site safety rules.
 Monitor all lifting equipment and accessories / tackles are certified and have a valid colour code
 Advise on the effectiveness and issue of the personal protective equipment and encourage /
enforce use on site
 Ensure permits to work are available and related conditions are being continuously implemented
 Ensure continuous safety watch is provided in hazardous areas and works categorized as
hazardous
 Attend safety and operational meetings and tool box meetings
 Ensure safety bulletins are updated and posted at prominent places
 Prepare and submit weekly and monthly safety reports
 Ensure accidents and near miss incidents are reported promptly and investigations are done and
recorded in case of an accident or near miss incident.
 Encourage the workers to follow the safe working procedures.
 Initiate and nominate workers for safety awards.

Subcontractors/Suppliers
 Comply with all HSE Requirements in conjunction with the approved HSE Plan
 Comply with training requirements of the project and IBSPL HSE Plan
 Ensure qualified and experienced personnel are assigned for job
 Report all accidents / incidents and near misses to IBSPL.
 Comply with the wearing of required PPE on the job
 Comply with the requirements of the safety meetings.
 Comply with the IBSPL’s HSE Management System requirements
 To participate in the Site’s safety promotional and training activities.
 To ensure that all plant operators are only employed on equipment for which they have been trained.
 To ensure that all operations carried out at each place of work and all plant, machinery and equipment
therein conform to the prevailing statutory requirements, and copies of valid statutory test certificates
approved 3rd parties are readily available for inspection.
 To attend the IBSPL Monthly Safety Committee Meeting.
 Responsible for safe work practices of workers and operations under his control.
 Responsible for the compliance with all the regulation and other relevant legislation within his scope of
work.
 Attend all Weekly and Monthly Safety Patrols.
 IBSPL to conduct internal audits every month to ensure HSE compliance by subcontractors and
suppliers.
6. ARRANGEMENTS FOR CONTROLLING SIGNIFICANT PROJECT RISK
6.1 Risk Assessments and Method Statement
The detailed method statements and risk assessments will be carried out on specific task prior to start
the activity. The Risk Assessment undertaken by IBSPL and their sub-contractors will identify particular
risks, the required management processes and other control measures such as Permit to Work, daily
supervision etc. Permits to Work will include but not limited to: Excavation, HV/LV Electrical Systems,
Confined Spaces, Work at Height, Hot Works, Work in Restricted Areas, Work is Shaft, Removing edge
protection, Scaffolding Permit. PTW Training will be given to those selected Permit Issuers and
Receivers; finally test will be taken to assess the competency. Hazard and Risk Assessment form is
attached with this plan.

6.2 Approval of Method Statements


Internal method statements will be prepared keeping a target of days prior to start the work. Any
Subcontractors must submit their Method Statements on agreed days prior to start the activity and
Project Manager or his designate will approve once it meets the IBSPL requirements. Project Manager
or his designate will prepare the method statements and submit for approval from client and consultants.
Method Statement and Risk Assessment will be reviewed periodically and/or when there are any
changes in operations. The Method Statements and the associated risk and controls will be
communicated through the Task Sheets to the workforce.

1. All outer edges of suspended floor slabs will be provided with handrail, mid rail and toe board.
2. Exclusive team shall be allotted to carryout fixing of edge protection in all areas under the control of a
supervisor depending on the requirement of the site. However, responsibility for fixing edge protections
shall rest with the concerned engineer / foreman in the areas in which they are working on day-to-day
basis.
3. Edge protection fixing crew shall be provided with special uniform (either a jacket or different color of
helmet) so that they will not be diverted for other activities.
4. The crew shall be given specific training and they must have experience of such work.
5. Binding wires shall not be used for tying edge protection pipes. Suitable couplers shall be used instead.
6. Sides of the staircase will be protected with a handrail and mid rail. To be installed progressively as each
level of stairs is completed.
7. The lift shaft entrances will be provided with suitable gates with offsets to prevent falls into the shaft.
PTW for work in shaft shall be followed and implemented
8. PTW shall be followed to remove any edge protection and opening coverings (Permit to Remove Edge
Protection/Barricades).
9. Whenever any maintenance work is taken up in the temporary electrical distribution network, caution
boards such as “Do not operate”, “Men at work” will be put up in the main boards and it will be locked for
further protection.
10. Defective scaffoldings, ladders or power tools, if found during the scheduled inspection, will be tagged
immediately and will be removed from service.
11. Any protection or barricade that is removed to gain access to the prohibited work locations to carry out
specific work must be replaced after the work is completed.
12. Information posters will be displayed in appropriate locations indicating the emergency telephone
numbers, location of fire extinguishers etc.
13. No smoking signs and hot work prohibition signs will be installed wherever required.
Suitable safety posters and stickers will be provided.

6.5 Scaffolding/Temporary & suspended work platforms


In order to ensure that a scaffold will meet the working requirements for which it is being erected, the
following requirements must be addressed: -

a) Scaffold must only be erected, dismantled or altered under supervision of ‘Competent Persons’.
Competent persons maybe someone formally appointed or one who has undergone training appropriate
to the safe provision and use of scaffold.

b) All the materials that are to be used in the scaffolding must be checked and inspected for safe condition
(by competent person) before they are used and if found to be defective, then taken out of use.

c) The bases on which the scaffold is erected sound and capable of distributing the load. Where necessary
ground / mud sills and adjustable base plates may have to be planned and positioned prior to erection of
the scaffold.

d) Scaffolding are not to be removed or interfered in any way, especially ties, guardrails toe boards, ladders
and rakers. Only the qualified scaffolders and competent person are qualified and responsible for such
tasks.

e) Damaged scaffold are not to be used while under erection and dismantling. Warning notices including
the Red tags are to be clearly displayed as appropriate.

f) Deficient scaffolding is to be reported to the Competent Person as soon as possible. Clear passageway
is to be provided when stacking scaffold.

g) The standards and ledgers must be in line, and the standards must be vertical, and they must not lean
away from the structure under any circumstances.

h) Scaffolding tag system to be followed.

6.6 Work at height/fall prevention

Work at height is any place of work where a person could be injured by falling from it and includes a place at or
below ground level. Safety nets where necessary shall be provided when working at heights where the use of
ladders, scaffolds, catch platforms, temporary floors, safety lines or safety belts is impracticable. The purpose is
to ensure that work at height is carried out safely and that persons performing the work do not
endanger themselves or other persons who may be exposed to the hazards of falling objects. This
procedure applies to all working at height on this project.

6.7 Temporary electrical distribution

a) All temporary electrical installations at site shall be provided with earth leakage circuit breakers (ELCB).
b) Earthling rods shall be used for proper earthling of equipment.
c) All Hand held portable electrical appliances, lighting and inspection lamps are routed through ELCB.
d) All temporary electrical installations shall be inspected by a competent person (Electrical in-charge) at least
once a month and a second copy of the certificate of fitness and inspection report will be forwarded to safety
officer.
e) Whenever any maintenance work is taken up in the temporary electrical distribution network, caution boards
such as “Do not operate”, “Men at work” will be put up in the main boards and it will be locked for further
protection.
f) Company procedure of lock out and tag out shall be followed, and permit to work system i.e isolation certificate
will be issued.

6.8 Electrically powered tools

a. All hand and power tools and similar equipment used at site shall be maintained in a safe condition.

b. When power operated tools are designed to accommodate guards, they shall be equipped with such guards
when in use.

c. Such tools must be inspected, examined and maintained by the electrician.

7. OCCUPATIONAL HEALTH AND PERSONNEL PROTECTION

7.1 Personnel Protective Equipment (PPE)

7.1.1 Personnel protection is essential to reduce human exposure to potential hazards associated with
work, which cannot be practicably controlled at source.

7.1.2 All employees will wear the correct personal protective equipment at all times.

Basic mandatory PPE include:


 hard hats (helmets),
 safety shoes,
 Hi Visibility vest.
 Safety Goggles (at work)

Other PPEs such as dust masks, respirators, face shield, ear defenders etc will be used wherever
required depending on the nature of activities and associated hazards.

7.1.3 Worn out or defective PPE will be replaced immediately. Personnel who willfully disregard these
regulations, or tamper with PPE to reduce its effectiveness will be subjected to disciplinary action.

7.1.4 The supervisor in charge will instruct their personnel for the correct application of PPE issued to
them and will ensure the usage.

7.1.5 All the employees are required to sign receipt of PPE and record will be kept available.

7.1.6 Subcontractors are responsible to supply their own PPEs. If they require PPE from IBSPL, the
requested PPEs shall be supplied to them and cost will be deducted from them.

7.2 Occupational Health

7.2.1 All our employees have been medically tested and certified for work in the construction sites.
Medical arrangements are in force with local hospitals / clinics for staff and workmen.

7.2.2 First Aid Kits will be available on sites to take care of minor injuries – cuts and bruises. Safety
Officer, Foremen, Site Engineer will be trained for handling the basic first aid services. These personnel
will be trained by an authorized Training Providers and necessary certificates obtained subsequently.
Designated First Aider/ Site Nurse will be identified on notice boards and shall be made familiar to
workmen.

7.2.3 Project shall have a designated site First Aider/Site Nurse who will be contactable at all times and
also responsible for maintaining basic First Aid supplies.
7.2.4 During all hazardous operations, continuous safety watch will be continuously available for any
possible emergency during the works.

7.2.5 During summer work, sufficient water and dehydration fluid when necessary will be supplied to
reduce the risk of heat stress amongst personnel.

7.2.6. First Aid station shall be fitted with minimum equipment and medicines as listed in the attachment
(Form IBSPL-HSE-F34, First Aid Supply List).

7.3 Toxic & Hazardous Substances – Control and Maintenance


7.3.1 Hazardous materials will be stored, handled and identified to comply with the precautions listed in te
Material Safety Data Sheets and existing COSHH Assessment system. Where ever applicable, local laws
will be adhered to.

7.3.2 The personnel will be trained about the risks and the applicable emergency procedures.

7.3.3 Supervisor will guide to those employees who handle hazardous materials, such information, and
instructions as are contained in the material safety data sheets and ensure that proper PPE is supplied
prior to starting work.

7.3.4 Spill kit will be provided on site and emergency control protocol will be followed.

7.4 Welfare Facilities


It is the responsibility of IBSPL Project Manager to ensure that suitable and sufficient welfare facilities
are provided. Facilities provided by IBSPL are as follows:
7.4.1 Maintenance, Cleaning and servicing of work site and toilet blocks
7.4.2 Toilets and Wash Block Facilities
7.4.3 Rest Areas
7.4.4 Drinking Water, Drainage and Disposal
7.4.5 First Aid Facilities
7.4.6 Waste Storage and Removal

8. ENVIRONMENTAL PROTECTION

8.1 General
8.1.1 IBSPL is committed to always minimize the adverse impact of the Construction operations on
the environment by ensuring that appropriate and applicable environmental protection controls are
taken.

8.1.2 The Project HSE Manager/Safety Officer and Project Manager will ensure that the impact of
their operations on the environment are assessed and minimized and that adequate arrangements
are in place to deal with potential environment pollution as applicable and agreed in the Contract
conditions.

8.1.3 All activities in the project execution will conform to the environmental standards.

8.1.4 Each supervisor and foreman will hold on a regular basis a toolbox talk with his workforce to
communicate the task instructions to the employee.

8.1.5 Project specific Environment Management Plan is attached with this plan.

8.2 Disposal of Waste and Hazardous Material

8.2.1 All waste will be identified, segregated, handled, and disposed off in accordance with the
prevalent Hazardous Waste Management Procedure as applicable for this project.

8.2.2 Consideration will also be given to the following:


 Material safety data sheet / acceptable practice.
 Municipality regulations or directives

8.3 Housekeeping / Tidiness


8.3.1 IBSPL firmly believes that a well-kept site promotes safety by minimizing hazards associated
with an untidy site. Housekeeping and segregating waste on a continuous basis would help minimize
impact of works on the environment. Cleanliness and orderliness will also ensure a more conducive
and safer environment for working. Waste will be segregated according to their nature (food waste,
general construction waste and hazardous waste) as per requirements.

8.3.2 Paramount importance will be given to site housekeeping and tidiness. Safety Officer will
carry out a monthly inspection. Housekeeping Rating form is enclosed for this purpose. The PM is
responsible to attend all non-conformity and rectify as far as reasonably practical.

8.3.3 Housekeeping within all work areas will be ensured by each supervisor. Sites will be kept
neat and tidy on a continuous basis and waste will be disposed off only at designated place/skip.

8.4 Dust Control


Necessary measures shall take to avoid undue emissions of dust during construction. This will mainly be
in the form of water spray on roads and stockpiles of dusty materials. In addition, vehicle emission
inspections should be carried out in order to identify vehicles that need service and repair including the
appropriate and relevant measures in their work procedures.

Materials with flying particles must be covered properly during transportation.

9. ACCIDENT / INCIDENT INVESTIGATION AND ANALYSIS


9.1 Injuries and Incidents and Reportable Injuries, Diseases and Dangerous Occurrences Regulations
(RIDDOR). Reportable Injuries & Accidents are also defined in PMC Contracts requirements.

9.2 The objective is to reduce the frequency of accidents and incidents by applying the knowledge learnt
from effective investigation, reporting, and analysis of past accidents data.

9.3 Immediate Reporting of all Incidents:


The Project Manager must be informed of any incidents on site by the quickest possible means who in
turn will report all incidents within 24 hours to the PMC using the Accident Notification Form.
Project Manager will report fatalities or major accident immediately, by telephone to Police Authorities
and simultaneously to the PMC.

The Designated First Aider will record accidents that result in first aid treatment being administered in
the Weekly First Aid summary.

Project HSE Manager/Safety Officer will investigate and report all accidents, dangerous occurrences or
accidents that are considered to fall into the category of RIDDOR. All “Near Miss” cases will be reported.
The PM may be called upon for advice and assistance. Copies of accident reports and investigation
results will be submitted to the head office HSE Manager, who will analyze the investigation then forward
the investigation with any comments to the PM who will decide whether further actions are required.

9.4 Arrangements for Subcontractor to inform IBSPL Company


Subcontractors are required to inform the IBSPL Project Manager / HSE personnel of any
accidents/incidents on the Project by the quickest possible means. Formal notification will be made to
the project HSE Manager within 24 hours using the accident notification form. The subcontractor must
also provide the following information, Full details and nature of injuries or the occurrence:
9.4.1 Accurate details of the treatment received, both on and off site.
9.4.2 Full details and nature of the incident.
9.4.3 Circumstances of the incident.
9.4.4 Direct reasons for the incident.

10. SELECTION AND CONTROL OF CONTRACTORS:


10.1 All contractors selected to work on this project will be subject to follow the IBSPL HSE Management
system procedures and project HSE Plan. They will receive a copy of this HSE Plan and sign to comply all
HSE practices and cooperate with principal contractor.

10.2 The staff meeting shall be held on a weekly basis. This meeting shall be conducted by the IBSPL HSE
personnel. The purpose of this meeting it to informally discuss the past weeks activities and next week’s
activities related to safety. Also site audit will be carried out randomly.
Suppliers of Materials.
10.3 Project Manager will communicate the Health and Safety requirements to the suppliers.

Selection of Plant
10.2 Plant required by the site will be procured through the agencies approved by IBSPL.Plant department is
only responsible to arrange such site requirements.
Shared use work equipment supplied for sub-contractor use
10.3 Shared use equipment must comply with the same requirements as all other site equipment. Suppliers
of shared use work equipment for the project will be selected, and the equipment used and maintained in
accordance with the local regulations and safety regulations.

11. LIFTING OPERATION PLAN


11.1 Procurement of Lifting Equipment:
The purchase of all lifting equipment and loose lifting gears shall be made keeping safety as one of the key
criteria of selection. All lifting equipment and loose gears must comply with the standards.

11.2 Third Party Inspection:


All lifting equipment and loose lifting gears shall be tested and examined by a Competent Engineer from third
party approved by local agencies.

11.3 Color Coding:


Loose lifting gears such as wire rope slings, chain slings, webbing slings, D shackles, power tools etc will be color
code as shown in the appendix of this plan.

11.4 Authorized Persons for Lifting Operations:


They are responsible for:
 lead the project crane team(s)
 delegate duties to suitably competent individuals or organisations
 consult with others having specialist knowledge
 Devote time and other resources to the role, as approved in each case by the Project Manager.
 stop all or any lifting operations if there is any doubt or concern over the safety of such
operations
11.4.1 Crane Supervisors:

12. CRISIS MANAGEMENT and EMERGENCY PROCEDURES

An Emergency Plan is necessary as a moral and legal obligation of management to protect the safety of people,
property and environment with a detailed and comprehensive procedure. Though the main emergencies are
identified such as drowning, Fire & Accident at work site etc. This will be reviewed during the project and a
detailed Emergency Plan and procedures will be developed and communicated for the implementation.

At any time it may be necessary for the safety of the personnel to evacuate the Project quickly. Preservation of
life and property is only possible if the evacuation procedure is effectively followed. At least two Assembly points
will be provided.

The names of contact person and telephone Nos. will be displayed in the Safety bulletin Boards available at site.

It will be the responsibility of the site management team to review and ensure awareness of emergency
procedures among all the site personnel.

Suitable training will be provided to all project personnel during various stages of the project.

Subsequent actions as listed below will be taken as an instruction from the Project/Site Engineer.
 Close all plant and equipment, if safe
 Stop all work and report to the nearest evacuation area / assembly area and await further
instructions
 Stop all equipment and vehicles safely
 Contact the Safety Officer and relay message to Company.

In an emergency all personnel will proceed in an orderly manner to the nearest safe assembly point. Normal work
will be resumed only after the all-clear signal is received

12.1 EMERGENCY PROCEDURE:-


This procedure outlines the action to be taken in the event of any emergency.
 Call is made to the emergency number
 Emergency response coordinator calls (by Phone or Mobile) to Emergency Number and gives
information to emergency service: who, what, where and when.

ALERT LEVEL-1 Originator informs Emergency Response Coordinator of First-Aid case. First Aid
treatment provided to the injured person by the appointed First Aider/Nurse. No further response within
this procedure.
ALERT LEVEL-2 Originator informs Emergency Response Coordinator of violence alert and that
police assistance has been requested. Emergency Coordinator prepares for escort of police to incident.
ALERT LEVEL-3 Originator informs Emergency Response Coordinator of incident.
 First Aid treatment provided at site by the appointed First Aider.
 Emergency Response Coordinator makes call to the emergency number 108/100
 Hospital staff retrieves injured party. Injured person return to site and report to his
immediate supervisor.

Nominated Site Engineer or Subcontractor (if any) involved will be responsible for the following:-
1) Assist Emergency Response Services with additional Plant, Equipment or Material as required.
2) Set up exclusion zone, take photo and sample of incidents, and gather all paper evidences.
3) Deploy HSE Team to barricade the area and take statements from eye witnesses and summarize
the findings.
4) When Emergency Response Services have left and given a ‘Go Ahead’ sign, deploy a Clean-up
Crew which could consist of Construction, Logistics and Public Relations
Precautions to be taken during emergency situation:
1) Control yourself and maintain calmness and immediately inform the coasts and borders security
department. After that deal with the situation giving preference for the most important thing at your
view.
2) Deal with the situation with the best solution you have until the rescue team reach you.
3) Send an urgent message in which you will specify your location and your situation in detail through
specified channels.
