I.

Our scope of services shall consist of the following;

1. Conduct visual inspection of the exposed structural elements.

2. In the course of our inspection, we shall require the assistance of one


of your senior representatives to inform the tenants or employees in
the area for us to be able to prepare our inspection schedule.

3. We shall conduct measurements of exposed structural elements with


the objective of preparing as-built drawings, without the steel
reinforcements.

4. Wherever possible, we shall determine the existing reinforcing bars


with the use of our Rebar locator.

5. Perform concrete core tests at strategic areas as well as Rebound


Hammer Test to determine the in-situ compressive strengths.

6. Should we discover any structural manifestations such as cracks,


deflections or other defects, we shall note them down in the form of
sketches indicating the type and magnitude of such defects. We will
take pictures of such observations.

7. We will perform gravity load as well as lateral load analysis and


investigate the stresses at various locations.

8. We proposed to complete the field inspections as well as analytical


investigations within Forty-Five Days (45) calendar days and submit our
report within Fifteen (15) days thereafter.

9. We will submit Seven (7) sets of our Final Report stating our
observations and findings.

10. Will issue yearly a Certificate of Structural Stability as required by LGU within
the 15-year liability period

11. Since the loads were analyzed as it was found in the time of Inspection, in the
event that a change of load was imposed, vendor will check if the area is still
adequate to cater the load.

II. Preparation of Plans and Specifications for Retrofitting and


Supervisory Works:

Section 1 - Preparation of Plans and Specifications for Retrofitting Works


The scope of works shall cover phases for the preparation of "For Bid
Plans and Specifications "including estimates for the successful execution
of retrofitting works among others the following:

1. Preparation and submission of final and complete sets of Plans


and specifications in standard 20x30 size blue print copies -Seven (7)
of copies

2. Preparation and submission of complete sets of Permit plans and


Specifications sign and sealed. Number of copies as per Building
Official requirement
3. Preparation and submission of Seven (7) sets (1- set each for WPS,
Site Copy and FM Copy) of copies of ''For Construction Plans and
Specifications".

Section 2 – Supervisory Services

The Supervisory Services shall include among others the following:


1. Making Periodic Inspection/attending meetings at the PROJECT site
in coordination with PLDT Project Manager, with such frequency and
duration as may be necessary to fully and carefully instruct the
Contractor and /or sub-contractors in carrying out the details of the
PROJECT and to pass upon the quality of both materials and
workmanship and see to it that the construction of the said PROJECT
shall proceed smoothly and in accordance with timetable or is done
strictly in accordance with every particular detail as approved by PLDT
or its authorized representative and condemn work and materials
which do not conform with contract documents and specifications.
The engineer shall personally conduct the inspection together with its
PROJECT Job Captain.

2. To check and approved samples, schedules, shop drawings and other


requirements, subject to and in accordance with the descriptive
information and provision of the Contract Documents and upon
concurrence of PLDT or its Project Manager.

3. The preparation of large scale detailed drawings for any


additional plans, changes and adjustment as may be deemed
necessary jointly by PLDT or its Construction Manager for the proper
execution of work.

4. The interpretation for PLDT or its Construction Manager and the


Contractor of the contents of the general Conditions and
Specifications such as drawing and other pertinent plans and
conditions.

5. The final inspection of the PROJECT together with the


representative of PLDT and other authorities concern.
6. Assist the Construction Manager to check and review "As Built Plans"
as prepared by contractor.

Exclusions:
1. Soil Boring test
2. Chipping works and restoration concrete coring shall not form part of this
scope.

In case our findings show any deficiencies or manifestations, we will submit a


separate proposal for our recommendations to retrofit / repair such defects.
Should there be no deficiencies found, we will submit a certification stating that
the structure is safe for occupancy.
It is understood that PLDT will furnish us a copy of all Architectural Floor Plans
for our reference. As soon as we receive the Architectural Plans, we will proceed
with our works to include preparation of Preliminary Structural Framing Plans
and Field Inspection.
