EMPLOYEE WAIVER FORM 

This work waiver serves as a written agreement between Angelica Insoy and Common
Prime Trading signed this 13th day of January 2023 by the employee, who
acknowledges and agrees to the terms provided below: 

TERMS AND CONDITIONS 

1. In consideration of the employee's insistence to continue to work despite her

condition of being pregnant for 3 months, the employee hereby acknowledges that her
decision is of her free volition and that the company did not force her to work.

2. The employee acknowledges and confirms that her decision to continue to work
despite her condition carries with it a potential risk for miscarriage and other health
problems, neither known nor reasonably foreseeable at this time. 

The employee is signing the waiver voluntarily, and by doing so agrees to release the
company from any liability for whatever happens to her within the scope specified
herein.
