Tutorial chapter 6
1. Define:
 Accident - accident result in physical damage and injury while near
miss does not.
 Near Miss - an event not causing harm, but has the potential to cause
injury or ill health.
 Incident - an unplanned, undesired event that adversely affects
completion of a task. Both accident and near miss are Incidents.

2. List purpose of accident investigation.


 To determine cause of accident
 To uncover problems
 To reduce the likelihood of accident
 Action items & improvement to prevent similar accident

3. Briefly explain 4 steps in accident investigation.


Isolate the Scene is All evidence can be collected, observed and recorded
otherwise Remove the injured worker from the scene. Next, Record All
Evidence is Create a permanent record as soon as possible. Certain types
of evidence may be easily damaged. Identify Witnesses There are three
types, namely primary witnesses, secondary witnesses and tertiary
witnesses. Witness Interview is Conducting interviews in the order of
primary witness, secondary witness and tertiary witness

4. Briefly explain three types of witnesses.


Firstly, is Primary witnesses being Eyewitnesses to the accident. Next,
Secondary witnesses are Witnesses who do not actually see the accident
happen but were in vicinity & arrive immediately or shortly after accident.
Tertiary witnesses are Witnesses who did not present at the time of
accident or afterward but may still have relevant evidence to present.
