Procedure For Accident Incident and Near Miss Reporting

Definitions
Accident – an unplanned, unwanted event that leads to injury, damage or loss.
Injury – where the unwanted, unplanned events lead to some sort of personal
injury.
Near miss – an unplanned, unwanted event that had the potential to lead to
injury or loss.
Roles and Responsibilities
Worker:
 Report accident
 Report incident
 Report near miss
 Report if found any dangerous occurrence.
Line Supervisor:
 Record the incident reporting.
 Take emergency service if necessary.
 Evacuate the person from the scene of accident.
 Provide ethe first-aid treatment.
 Investigate the immediate and root cause to prevent reoccurrence.
 Report to top management about the incident.

HSE Officer
 Investigate all incident.
 Find out the immediate.
 Organize the accident investigation team.
 Investigate the accident.
 Find out corrective action & preventive action.
 Share the incident details to workers during the tool box talk.
 Prepare incident investigation report.
 Ensure sufficient PPE’s available at the company.
Procedure
Accident Investigation
 Gather actual information about the event using the accident/incident
investigation format.
 Analyze that information and draw conclusion about immediate,
underline and root causes.
 Identify suitable control measures.
 Plan the remedial action.
 Report using accident/incident investigation format.
 Review the corrective action and preventive action.
 Record the report.
 Communicate to team and client.

Near miss recording procedure


 Find the near miss deviations in the company.
 Report it to the line supervisor and HSE officer in-charge.
 Record the near miss deviation in near miss recording format.
 Find the root cause analysis.
 Take corrective action and preventive action.
 Maintain as record.
 Communicate to team head and client.
