Definitions
Accident – an unplanned, unwanted event that leads to injury, damage or loss.
Injury – where the unwanted, unplanned events lead to some sort of personal
injury.
Near miss – an unplanned, unwanted event that had the potential to lead to
injury or loss.
Roles and Responsibilities
Worker:
Report accident

Report if found any dangerous occurrence.
Line Supervisor:
Record the incident reporting.
Take emergency service if necessary.
Evacuate the person from the scene of accident.
Provide ethe first-aid treatment.
Investigate the immediate and root cause to prevent reoccurrence.
Report to top management about the incident.
HSE Officer
Investigate all incident.
Find out the immediate.
Organize the accident investigation team.
Investigate the accident.
Find out corrective action & preventive action.
Share the incident details to workers during the tool box talk.
Prepare incident investigation report.
Ensure sufficient PPE’s available at the company.
Procedure
Accident Investigation
Gather actual information about the event using the accident/incident
investigation format.
Analyze that information and draw conclusion about immediate,
underline and root causes.
Identify suitable control measures.
Plan the remedial action.
Report using accident/incident investigation format.
Review the corrective action and preventive action.
Record the report.
Communicate to team and client.
