Traffic Accident – A shuttle minibus got sideswipe collision with a Truck, Bangkanai Asset
SUMMARY:
Date & Time of Incident : 11th January 2021 at 11:07 WIB
Asset : Bangkanai
Location : Jl. Trans Kalimantan Sikui Kandui KM 50
Incident Classification : Serious Traffic Accident
Incident Risk Rank : C1 x L4 = 4 (MEDIUM)
DESCRIPTION:
A shuttle minibus (DA 7089 CD) with 5 passengers was heading to Palangkaraya City from Muara
Teweh. This trip is a regular commuting for crew change (Off-duty travel). When a minibus got through
Trans-Kalimantan Sikui Kandui Street KM 50, got sideswipe collision with a truck (outsider) that coming
from opposite direction. Then minibus’ outside mirror was folded, hit & broken the window glass pane
on the driver side. No injury caused by this incident.
For additional information, a minibus speed was around 75 km/hour. The road condition was relatively
narrow. The damage cost approximately US$750.

Location : Trans Java Highway ( Solo-Kertosono) KM502

DESCRIPTION:
A shuttle bus rent by Company (BA 7013 ZAA) with 2 passengers was heading to Jakarta City from East
Java via Trans Java Highway. This trip is a regular commuting for crew change (On-duty travel). When a
shuttle bus got through Solo-Kertosono Area KM 502, suddenly the rear tire on the right side got
damage (top layer was peeled off). Driver was still able to manage a shuttle bus, decrease a speed &
then pulled over to check the condition. No driver & passengers injury caused by the event.

Traffic Accident – Light Vehicle BG1027OR Slipped to The Road Side during Trip Soka - Pendopo
DESCRIPTION:
During convoy road trip from Soka to Pendopo, Light Vehicle BG 1027 OR at Bukit Tudung crossroad, driver
took the wrong direction since he was left behind the convoy and honked by LV behind it. Driver surprised
and had chance to see the rear view mirror but he lost focus, accidentally the LV slipped to the road side.
The driver was unfamiliar with the route since he is in new a new hire in SSB. Then driver stopped to check
the LV and informed to dispatcher.
There is no person injured caused by the accident but causing minor damage on the left front side LV
(covered by insurance). The LV was puled out from the road side and the driver temporary suspended to
follow the next procedure.
