Dear Teacher Juvelyn,

This is to authorize Ms. Pery (Joy) Dela Cruz Comighud, to get the learning packets/materials/modules of
my grandchild Mathew Alexis Duran, Grade 5- Star, in my behalf. We would also like to inform you that
we are affected by Typhoon Ulysses and some of his books, notebooks, notes and things from Grade 4
to Grade 5 were submerged to flood. But they are still with us and we are willing to return the Grade4
books if needed but it is all wet and flooded. We cannot pay it since we are in the process of recovering
this traumatic experience.
