AFFIDAVIT OF DESISTANCE

I, Mary Grace Antenero Andig, of legal age, Filipino, single, and a

resident of Purok 7, Barangay Matam, Katipunan, Zamboanga del Norte,
Philippines, after having been duly sworn in accordance with law, hereby
depose and say:

1. That I am the private complainant in a case entitled

“Republic of the Philippines vs Juan Dela Cruz” with Docket No.
12345 against Juan Dela Cruz for Slight Physical Injury which
case is now pending Preliminary Investigation before the
Office of City Prosecutor in Dipolog City.

2. That I am voluntarily desisting and no longer interested for the

further prosecution of the case against respondent because after
due analysis and assessment, I decided to forgive the respondent
since he is a friend of mine for 10 years and what was happened
was due to my provocation.

3. Accordingly, I hereby formally desist or withdraw my complaint

and manifest that I am no longer interested in the prosecution of
the above-mentioned case;

4. That I am voluntarily executing this affidavit of desistance to attest

to the truth and veracity of the foregoing statements.

        IN WITNESS WHEREOF, I hereby set my hand this 26 th day of September

2023 at Sicayab, Dipolog City, Zamboanga del Norte.
