     OWWA MEMBERSHIP APPLICATION
     WALK IN ANY REGIONAL WELFARE
    OFFICE
    REQUIREMENTS:
     OWWA Membership Form.
     Copy of Passport (emphasis on
    the data page with photo)
     Employment VISA
     Valid Employment Contract.
     Membership fee.
    NOTE: OFWs can also renew their OWWA
    membership in the Philippines with the
    support of family members (first degree) by
    completing the requirements to their
    nearest OWWA Regional Welfare Office.
