hereinafter referred to as the “MAKER”, hereby undertake to pay to EDRALIN
BERROYA, hereinafter referred to as the “PAYEE” the sum of

_____________ with interest at the rate of ten percent (6%) per
annum until fully paid.

In the event of default, when the maker fails to pay in two (2) consecutive
monthly installments, the whole amount obligation becomes immediately due
and demandable. The payee shall be entitled to all rights and remedies of a
creditor as provided for under the Civil Code of the Philippines.
Any unpaid amounts shall be subject to the payment of legal interest.

In the alternative, when payment of legal tender is not possible, the maker
hereby undertakes to execute any necessary document to assign to the payee
movable/immovable properties to cover the amount of the debt.
