CHECKLIST FOR

VISITOR VISA APPLICATION - VIETNAMESE

You must lodge your application at the New Zealand Visa Application Centre (VAC) in Ho Chi Minh City. For more information about
the VAC visit: www.vfsglobal.com/newzealand/vietnam. It’s important you provide all the relevant information requested below so
we can process your application promptly. Application forms are available from our website: www.immigration.govt.nz/forms.

If you have been granted a visitor visa to New Zealand in the previous 3 years and there have been no changes to your current
circumstances since your last application, then you do not need to provide the documents marked * for lodgement purposes.

If you belong to a group of 4 to 20 persons and the group is travelling for the same purpose on the same flights and all group
members will be travelling to and leaving New Zealand together, the group could be eligible for a group visa. Group visa applicants
are required to complete a Group Visa application form INZ 1021 together with other form and supporting documents listed below.

Alternatively you may be able to apply on-line for some visa types. You can access our on-line visa application service through our
website: www.immigration.govt.nz.

• When providing original documents with your application, ensure they are accompanied by a standard photocopy.
• All forms must be properly completed and each signature section signed. Mark N/A to question(s) not applicable to you.

Quý vị phải nộp hồ sơ cho Trung tâm tiếp nhận hồ sơ xin visa New Zealand (VAC) tại TP Hồ Chí Minh. Tham khảo trang web
www.vfsglobal.com/newzealand/vietnam để biết thông tin về VAC. Để thuận tiện cho việc xét hồ sơ nhanh chóng, quý vị nên

Nếu quý vị đã được cấp visitor visa trong vòng 3 năm trở lại đây và hoàn cảnh hiện tại của quý vị không có gì thay đổi kể từ lần
nộp hồ sơ xin visa trước, quý vị không cần phải nộp những giấy tờ bắt buộc được đánh dấu * để được chấp thuận nộp hồ sơ.

Nếu quý vị thuộc một nhóm từ 4 đến 20 người và nhóm sẽ đi New Zealand cho cùng một mục đích trên cùng một chuyến bay và
tất cả thành viên sẽ đến và rời khỏi New Zealand cùng lúc, nhóm có thể xin “Thị thực Nhóm” - “Group visa”. Tất cả các thành viên
của nhóm được yêu cầu điền chung một đơn INZ1021 cùng với đơn xin thị thực thông thường và các giấy tờ đi kèm được yêu
cầu bên dưới. Quý vị có thể tham khảo thông tin về “Thị thực Nhóm” - “Group visa” trong trang web của Sở Di trú bằng cách tìm
kiếm theo từ khóa “group visa”.

Hoặc quý vị cũng có thể nộp hồ sơ trực tuyến cho một số loại thị thực. Quý vị có thể xem dịch vụ nộp hồ sơ trực tuyến qua trang

Xin lưu ý:

Khi nộp giấy tờ bản gốc kèm hồ sơ, quý vị vui lòng nộp kèm theo một bản sao.

 Two recent passport-sized color photographs (must be less than 6 months old) of each person included in the application,
attached to the application form

 Work Reference letter and leave approval from the employer, or Evidence of company’s ownership and business tax

your partner/boyfriend/girlfriend (account of the relationship and other evidence e.g. marriage certificate, birth certificate
of your children, correspondences, communication records, photos, etc.)

 Police certificate from country of your citizenship and from any country in which you have lived for 5 years or more since

information and documents on file.
