2.2 Attention
Please read this Manual thoroughly. Although Allengers devices comply with the related
Safety Requirements. This manual provides all information necessary for a correct use and
the warnings related to danger associated with X-Rays Generating Units.

Allengers, is not to be held responsible for,


• Use of the equipment different from the intended one as described in this Manual;
• Damages to the unit, to the operator, to the patient, caused either by wrong installation
or maintenance procedures, different from those described in this Manual supplied
with the unit, and by wrong operations;
• Mechanical and/or electrical modifications performed during and after the installation,
different from those described in the Manual.

Allengers Qualified Engineers must only perform any technical intervention. If so


required, only the authorized personnel can remove the covers and has access to the
internal circuitry of the equipment.

Warnings:
• This device has not been designed to be used in environments where vapours,
anesthetic mixes flammable with air, or oxygen and nitrous oxide can be detected.
• Before cleaning the device, please disconnect it from the AC Mains supply.
• Wherever necessary, use the suitable accessories, such as the Leaded Aprons, Lead
Goggles, and Thyroid Guards to protect the Patient, User & Support Staff of OT from
radiation exposure.
• While performing the radiography, no one, apart from the Operator and the Patient,
must remain in the room.
• Though this unit has been designed with a quite acceptable protection level from
electromagnetic interference, it is advisable to install it at a certain distance from
electrical energy transformation rooms, from static continuity units, from portable
receiving-transmitting units and for Cellular use. Cellular Telephones are only
admitted at a distance of more than 1.5m from any component of the device. Other
medical instruments and devices that must be used in the same installation area of the
unit must comply with the Electromagnetic Compatibility rules in force. Non-
complying instruments, of which the poor immunity from electromagnetic fields is
well known, must be installed at least 3 m away from the equipment and supplied by a
different electrical line.
• The equipment must be OFF while using devices such as electrical scalpels or similar
instruments. The equipment automatically performs some self-test functions and
therefore is able to detect some anomalous conditions. However, whenever there are
doubts on the performance of the equipment (e.g. images too clear, too dark or with
artifacts), the activity must be suspended and an Allengers Qualified and Authorized
Engineer must be contacted.
• During the use of the equipment, the operator must comply with the local work Safety
Regulation, in particular with the ones referring to the use of X-Ray Equipments.
• Avoid to use the CFL Lamps in O.T. as some of the CFL Lamps generate UV
Spectrum (>700 mm) which some times hamper the performance of electronic
circuits.

Environmental risks and displacement

The equipment contains in some of its parts, materials that at the end of the unit’s life, must
be recycled at an approved location. Particularly the device contains the following materials
and/or components:

• Central Unit and Control Console: Iron, Copper, Aluminum, Non-biodegradable


Plastic Material, Glass-Resin for Printed Circuit Boards (PCB).

• Tube Head Assembly: Tube Head Assembly of this X-Ray Equipment contains High
Voltage Tank, X-Ray Tube and Oil filled alongside, Printed Circuit Board (PCB) and
Lead used for controlling Stray Radiations.

Allengers is not responsible for the disposal of the equipment performed by the user and
for the costs related to this intervention.

It is assumed by the manufacturer and distributors of this equipment that all persons
responsible for the operation of this equipment are aware of danger and excessive exposure to
X-Radiation and this equipment is sold with the understanding that Allengers Medical
Systems Limited, their agents and representatives have no responsibility for injury or
damage which may result from exposure to X-Radiations.

Recommendations to reduce Radiation Exposure


The main objective of radiation protection procedures in an X-Ray Diagnostic Installation is
to limit the Radiation Exposure arising from the use of the Diagnostic Equipment as low as
possible so that the radiation doses received by the radiological personnel and general public
never exceeds the maximum permissible dose limits recommended by ICRP (International
Commission on Radiation Protection).

a) The radiation exposure to the patient should be the minimum exposure required to
produce images of good diagnostic quality. Use radiation with care, caution and at a
rate which is ALARA - ‘As Low As Reasonably Achievable’

b) The speed of the film, or screen and film combinations should be of fastest speed
consistent with the diagnostic objective of the radiographic examination.

c) When a patient or film must be provided with auxiliary support during radiation
exposure:

(i) Mechanical holding devices shall be used when the technique permits.

(ii) Individuals may be permitted to hold the patient when absolutely


necessary and no individual shall be used routinely for this purpose to the
exclusion of others who might share the task.

(iii) If a human holder is required, the holder shall be positioned such that no
part of body shall be struck by primary X-Ray beam unless protected by at
least 0.5 mm lead equivalent and shall be protected from direct scatter
radiation by protective lead apron of not less then 0.25 mm lead
equivalent.

d) Gonadal shielding of not less then 0.25 mm lead equivalent shall be used for patients
who have not passed their reproductive age during radiographic procedures in which
gonads are under primary beam, except, in cases where shielding would interfere with
diagnostic procedures.

e) Doors of the Room/OT where X-ray equipment is installed should be closed before
making exposures.

f) The X-ray beam should not be directed towards doors or windows of the room, or
towards control panel or darkroom walls unless no other geometry is possible.

g) Obey radiation protection rules pertaining to time, distance and shielding.

h) Prefer low exposure settings to high.

i) Use Collimator to set minimal useful field size; keep Image Intensifier in close
proximity to the patient.

First Aid In Case of Electrical Shock

1. Switch OFF the Power Supply: If this is not possible, protect


yourself with dry insulating material and pull the victim clear off the
conductor. Do not touch the victim with your bare hands until he/she
is clear off the conductor.

2. Place the victim in supine position.

3. Keep the air passage clear by turning the head to one side, opening
the patient’s mouth and cleaning it of water, saline, mucus or blood,
a lot of which might have accumulated in the back of the throat.

4. If the jaw is rigid, try to force the mouth open by pressure on the
gum behind the last molar tooth of the lower jaw. When the upper air
passage is cleared, tilt the head backward and force the jaw forward
from the angles of the jaw in front of the ears. This would prevent
mechanical obstructions to the upper air passages.

5. Hold the chin up and forward with one hand and pinch the nostril of
the victim with other.

6. Take a very deep breath and apply your mouth to that of victim and
blow into his mouth until the chest of the victim moves up indicating
filling of the lungs (Never allow the chin to sag).

7. When the chest has moved up, withdraw your mouth and allow the
chest to sink back.

Have someone else sent for a Doctor.

Keep patient warm and loosen his/her clothing.

Do not give liquids until the patient is conscious.

3. Introduction

3.1 Introduction to Machine

Keeping in view the ever-increasing requirements of Surgeons in the field of Orthopaedics,


Urology, Cardiology, we at Allengers continue to develop products and services in line with
their needs. Allengers HF-46/49 is such an outcome of our efforts which promises to deliver
superior image quality, ease of operation & position, safety to patient & operator, increased
life of vital components of equipment and user interactive self diagnostic capabilities for
increased up-time of equipment.

3.2 Safety
X-ray and fluoroscopy are indispensable accessories for orthopedic procedures. Their use is
beneficial to a patient but exposes the surgeon to radiation hazards. Majority of orthopedic
surgeons consider these hazards insignificant either because of bravado or ignorance. The
primary concern has been possibility of malignancy and infertility. The most worry is about
the thyroid, marrow organs (liver and spleen), gonads, hands (skin) and the eyes (lens).
Image intensifier is becoming popular but standard X-Ray unit is still is the workhorse.
Although fluoroscopy reduces the radiation to far below acceptable limits, no author
advocates decreased protection and cavaliser use of the modality like direct handling of the
tube and placement of the hands directly in the field.

The radiation-induced effects are described as stochastic and non-stochastic. A non-


stochastic effect is one that increases in severity with increasing absorbed dose (e.g.,
decreased sperm production, skin erythema, and lens opacification). It occurs only after
relatively high exposures to radiation. In a stochastic effect, the probability of occurrence
increases with increasing exposure; however, the severity is independent of the magnitude of
the absorbed dose (e.g. cancers and genetic effects). This is the principal hazard of exposures
to low doses of ionizing radiation.

The objective of a radiation protection program is to prevent the occurrence of non-stochastic


effects and to limit the risk of stochastic effects to a reasonable level. In this issue two
experts have highlighted the ill effects of the Roentgen rays on living tissue and enunciated
the safety principles. All orthopedic surgeons should use well collimated X-ray machines
and make use of intensifiers in image memory mode in preference to live fluoroscopy, stand
as far away from the tube as possible and protect themselves with lead aprons, lead-lined
eyeglasses and thyroid shields.

3.3 Recommendations & Practices

Recommendations
To reduce Radiation Exposure

Use radiation with care, caution and at a rate, which


is ‘As Low As Reasonably Achievable’
(ALARA)

1. Obey radiation protection rules pertinent to


time, distance and shielding.
2. Utilise pulsed fluoroscopy mode to reduce
exposure by a factor of 3 to 10.
3. Prefer low exposure settings to high and
reduce fluoroscopy exposure by 50%.
4. Use collimator to set minimal useful field
size; keep image intensifier in close
proximity to the patient.
5. Stand on the image intensifier side of the
fluoroscopy (see figure I)
6. Step back during fluoroscopy

It is not possible to quantify the radiation received by


hands which are 50 times less sensitive than rest of
the body. Undue fear of radiation should not hold
back medical care.

Good practices
For optimum radiation protection

• Use equipment certified safe by the


Regulatory Board.
• Maintain equipment in optimal working
condition
• Use good quality radiation protection devices
certified by the Regulatory Board
• Use radiation only when required
• Use it as sparingly as possible
• Use image storage devices during
fluoroscopy
• Stand as far away from the primary beam as
possible (see figure I)
• Use TLD monitoring

4.1 Prescribed Usage
Allengers HF-46/49 is a mobile C-Arm X-Ray I.I.T.V. System for Fluoroscopy &
Radiography. This equipment is to be used only by an Orthopaedic Surgeon, Urologist,
Gastroenterologist or a Cardiac Surgeon. Its unauthorized use can cause injury or damage.

5. System Specifications

5.1 Brief Description of Principal Modules


Allengers-HF-46/49 is a Mobile C-Arm Image Intensifier X-Ray T.V. System. It consists of
the following Main Assemblies:
1. Control with Panel
2. C-Arm
3. Image Intensifier with CCD Camera & Optics
4. X-Ray Tube Head Monoblock
5. Image Memory
6. Trolley with 2 Monitors

OPTIONS:
1. Servo Voltage Stabiliser
2. Thermal Printer

The brief description of various Assemblies is mentioned below:


1. Control with Panel

The control houses the Invertor Assembly of the X-Ray Generator alongwith other control
& command circuitry. All parameters related to operation of X-Ray generation can be
selected & regulated from the Control Panel. Various display windows indicate the
parameters. The detailed functions of each Key on the control Panel is given in the separate
section of this Manual.
The control panel uses a flat Panel Keyboard, which makes it easier to clean & disinfect.
2. C-Arm
The C-Arm is mounted on the carriage attached to the control panel. On one end of the C-
Arm, Image Intensifier with CCD camera & optics is mounted and on the other end the X-
Ray Tube head is fixed. Both these devices are centrally aligned with each other. The C-
Arm also carries the electrical wiring in its cavity positioned in the back of C-Arm. This
wiring makes interconnections between control panel to I.I. Tube & X-Ray Tube head.
3. Image Intensifier with CCD camera & optics

The Image Intensifier head consists of Image Intensifying Tube of 6” or 9” Diameter (as per
customers order). The Power Supply of the Image Intensifying Tube is also housed inside
the Projected rectangular portion of the Image head.
On top of Image Intensifying Tube, a CCD camera with suitable optics is mounted. The
camera is covered by a Round Cap on I.I. Tube.

4. X-Ray Tube head Monoblock

The X-Ray Tube head Monoblock contains a Double Focus Stationary Anode X-Ray Tube.
Also housed are filament transformers, high voltage transformer & other related parts and
components. The X-Ray Tube is filled with Transformer oil for providing Insulation
Strength & for cooling of X-Ray Tube. The Tube head is totally sealed to prevent any
leakage & air to get in.
The X-Ray Tube head Monoblock emitts the X-Rays when Exposure is initiated from
control panel or foot switch.

5. Image Memory

Digital Image Processor with Keyboard and Pointing device.


Analog Memory with Temporary or permanent storage of image.
The main function of the Image Memory is to store the X-Ray Images of the Patient.

6. Trolley with 2 Monitors

This is a Mobile Trolley on which Image Memory, 2 Monitors & other optional devices like
Thermal Printer can be placed.
A 4 way Terminal Block is mounted on the back of Trolley from where the Electrical
Supply to Image Memory & Monitors is provided.
High Resolution 15” or 17” Monitors are provided. With Digital Image Processors the
Monitors provided are 17” flat screen PC Monitors.

OPTIONS

1. Servo Voltage Stabiliser


It is highly recommended that a Servo Voltage Stabiliser of atleast 2 KVA Rating is
used to give stabilized voltage of 230 volts AC ± 2% to the Mobile C-Arm.

2. Thermal Printer
A hard copy print for Patient’s Image can be taken using Sony Thermal Printer.

EARTHING

It is strongly recommended that independent EARTHING (at zero


potential) is available in the power socket in the Operation Theatre
where the machine is to be installed. Improper EARTHING can result
in poor image quality (i.e. snow in image can be there).
Proper EARTHING not only enhances the performance & reliability of
the equipment, it is safe for the operating staff as well.

Control Keys Description


1. MACHINE ‘ON’ KEY:
Pressing this key turns ‘ON’ the machine

2. MACHINE ‘OFF’ KEY:


Pressing this key turns ‘OFF’ the Machine. In ‘OFF’ mode the LED on the “OFF’ Key glows
and when ‘ON’ key is pressed this LED stops glowing.

3. FLUORO/RADIO MODE SELECTION KEY:


Pressing this key enables the operator to select the fluoroscopic or radiographic mode operation
of C-Arm. Respective LED glows when a mode is selected. By default the machine is ON in
fluoroscopy mode.

4. RADIOGRAPHIC EXPOSURE KEY

Pressing this key makes radiographic exposure & buzzer starts beep.

5. BODY PART CHANGE KEY

By pressing this key operator can change the parts of patients body i.e. Finger, Head, Skull.

6. View in APR Mode

It is used for APR to change the view (LAT, OBL) in this machine.

7. mAs DECREMENT KEY:

This key is used to increase/decrease the mAs in Radio mode.

8. REBOOT KEY
This key is used for reboot the machine. (This function not used in this machine)

9. EMERGENCY EXPOSURE KEY


This key is used for making an emergency exposure. (This function not used in this machine)
These keys are used to do fluoroscopic exposure in case of emergency i.e. in case of an
interlock is glowing on display during exposure but still fluoroscopic exposure is required. To
do emergency exposure, press Ready key initially and then simultaneously pressing the
emergency exposure key.

10. FLUORO EXPOSURE KEY:


This key is used to make a fluoroscopic exposure, it is parallel to the foot switch and function is
same as foot switch.

11. HIGH DEFINITION EXPOSURE KEY:


When high definition mode is enabled; this key initiates the High definition (CINE)
fluoroscopic exposure.

12. KV INCREASE/DECREASE KEY:


This key is used to increase/decrease the KV.

13. I.I. ZOOM KEY (OPERATIONAL WITH 9” I.I. ONLY):


This key enables the operator to select the Image field of a 9” I.I. Pressing repeatedly selects the
9”, 6.5” and 4.5” fields in sequence (indicated by LED).

14. BUZZER RESET KEY


This key is used for reset the buzzer after 300 seconds of time.

This key Resets the 300-second cumulative timer to zero after 300 seconds of total fluoroscopic
exposure time. After 300-second fluoro exposure, a buzzer sounds and exposure is terminated.
By pressing this key fluoro time is reset and fluoroscopy can be resumed.

15. AUTOMATIC BRIGHTNESS STABILIZER KEY (ABS):


This key enables the automatic brightness control mode. In this mode, the machine selects the
most optimum factors of fluoroscopic KV & mA automatically depending upon the thickness of
area under observation. When ABS is ON the indicator with this key is glowing.

16. FLUOROSCOPY MODE KEY:


Pressing this key selects the desired mode of fluoroscopy. There are three different modes of
fluoroscopy available. By default continuous mode of fluoroscopy is selected. Pressing this key
once selects single pulse fluoroscopy mode in which exposure takes place for single pulse of
short duration. Again pressing this key selects pulsed mode of fluoroscopy in which exposure
takes place in pulses instead of continuously so that exposure dose to patient is less.

Model: Allengers – VAM (100 Frame with Permanent Storage)


The Allengers HF 46/49 is supplied with an electronic image memory unit for X-Ray Fluoroscopy. It
captures video frames and displays them on video monitors. This helps to avoid continuous exposure
of the patient to X-Rays for prolonged period during fluoroscopy examinations.

The Features of Image Memory Unit (100 Frame VAM) are as follows:

XC-V series, X-ray image memory / controller is a state of the art fluoroscopy image memory and
controller to capture, process and display X-ray images. This helps to avoid continuous exposure of
the patient to X-rays for prolonged periods during fluoroscopy examinations. Advanced features
include real time rotation, Non volatile permanent image storage and Image backup directly to USB
flash drives or over LAN to a storage PC.

Display:
• Supports dual VGA monitors for flicker free display or CCIR monitors (selectable by switch
in the back panel)
• Real time Image Rotation and Mirror
• On Screen Display (OSD) text on both monitors

Image Capture:
• High Resolution Image Capture at 768 pixels X 576 lines.

INSTALLATION

Before installing the memory unit, connect the video output of the C-Arm directly to a CCIR video
monitor. Apply power and activate the foot-switch to check whether the C-Arm and monitor are
working properly.

Rear Panel Connections

The function of each connector at the rear panel of the unit is as follows.
1. LAN connector: This is the Ethernet interface to a PC.
2. RS232C Connector: This connector is for factory use.
3. Video-in BNC Connector: The video signal (CCIR 625lines / 50Hz) input connector
(connected from C-ARM camera output).
4. X-Ray Interface connector: This 25 pin D-Sub (Female) connector is connected to the
Relay module (interface option-1) or can be used to control image capture directly in
interface option-2.
5. Video out BNC connectors: These two are CCIR standard video output connectors.
6. CCIR/ VGA selection switch: This selection switch is used to select the Video output
standard as VGA or CCIR.
7. Video out 15 - Dsub Connectors: These two are VGA video out connectors. (Any standard
VGA monitor can be used)
8. Power connector: AC Mains input power connector. (170-240VAC-voltage range, 50/60Hz)
POWERING UP & BASIC FEATURES OF XC-V
When system is switched ON XC-V boots up and ‘Blinking mirror LED’ indicates the booting
operation, which may take up to 30 seconds. Subsequently images are loaded from internal non-
volatile storage, and message “Use a Key to Skip Images” appears on the monitor. Pressing any key
of memory will by pass the loading of previous stored image and boot up process becomes faster.
Note: If the system fails to boot up, a continuous beep is heard.
After successful booting, Monitor1 shows a bar pattern. The multi gray bar pattern is used to do any
brightness and contrast adjustments on the monitor.

Video Monitors

XC-V supports 2 monitors as described above.

LIH monitor (Monitor 1) is used to display Live/ LIH - captured images). All images saved by foot
switch operation (in LIH mode) are saved to this monitor.

Monitor-2 is used to display any of the 100 images from the built in image memory of the unit.

Image selection on Monitor-2: Under normal operation of the memory unit, the rotary dial can be
rotated ( left / right) to select images on the Monitor-2 and the image number is shown on top of the
screen. (Image numbers 1-100 & REF is displayed on top of the monitor)

SAVE & RECALL operations

(LIH monitor <> Monitor-2, image transfer operations)


At any time, the images on the LIH monitor can be transferred to Monitor-2 by pressing the SAVE
key.
The image can be transferred back from Monitor-2 to LIH monitor by pressing the RECALL key.

“REF Image KEY“

REF image is a reserved location on the Monitor-2.


Advantage of this reserved memory is that,
1. It can be displayed on Monitor-2 any time by pressing the REF key.
2. It is not over written in pulse mode of capture operation.

Non volatile (Permanent) memory operation


All images transferred to Monitor-2 are also automatically saved to an internal Non-Volatile memory.

On power up, all these (100) images are automatically restored from the non-volatile memory. Also,
after power failures, the images are restored automatically without any corruption.

(In pulse mode this permanent memory storage operation has to be initiated manually)

Image BACK-UP operation

It is always necessary to back up the images stored in XC-V units. For this, two options are available
1. Via USB 2. Via LAN

In USB option:

A USB pen drive can be connected to the USB connector on the front panel of the unit. The STORE
key can be used to save any images displayed on LIH-Monitor to the pen drive.

The images stored in the pen drive are sequentially numbered (File-No), along with time stamp.
Storing is always done to next sequence number, so as not to affect already stored images.

READ key can be used to read images from the pen drive to the LIH – Monitor. The image FILE-
No to be READ from the pen drive can be edited / selected by the user ( the time stamp is also
displayed along with FILE-No when image is read from the pen drive)

In LAN option:

For transfer of images to / from a PC over LAN, the XC-IMAGER s/w application, should RUN on
the PC, and XC-V should be connected to the PC.

Refer s/w reference guide for details.

Transferring images to LAN connected PC:

The STORE key can be used to save any images displayed on LIH-Monitor to the LAN connected
PC and these images appear on the PC monitor.

The images sent to the PC are saved in the directory set by the user.

Reading an image from a LAN connected PC:

READ key can be used to read images from the PC to the LIH – Monitor. The image to be READ
from the PC can be selected by the user from a directory set by him.

PULSE-Mode (CINE mode):

Pulse mode is used to capture images in succession, so as to record a sequence of images in a


procedure.

Note: The pulse-captured images are not automatically saved to the internal Non-volatile
memory.
After pulse capture, when foot switch is released, “Press save for Backup” message appears on the
screen. Press SAVE key to backup the pulse captured images to non-volatile internal memory. (A
new foot switch or key press operation skips this SAVE operation)

Description of Front panel

On the front panel, the keys are grouped into four functional groups: (from left to right) the
CAPTURE, PROCESS, USB/LAN and MEMORY groups. A valid key press produces a “short beep”
sound from the buzzer, while an invalid key produces a “long beep”. In addition, an infrared (IR)
detector is present on the front panel.

ROTARY DIAL:

A rotary dial is present on the right side of the front panel. This is used as a flexible input device to
change images on the REF monitor, adjust rotation, or edit parameters in the system configuration.

The Rotary Dial is similar to the tuning control knob on a Radio, and can be rotated clockwise or anti-
clockwise. The Dial could also be depressed (CLICKed) to perform certain operations. Each of these
operations (turning clockwise, anti-clockwise or clicking) has special meanings depending on the
operating context.

USB connector: A USB2.0 full speed connector is present on the front panel to connect pen drives
for storing / reading captured images

INFRA-RED (IR) REMOTE

The keys on the IR remote are essentially the same as those on the front panel. The main differences
are: on the IR remote, the Rotary Dial functions are carried out by the four arrow keys at the bottom.
The left and right arrow keys on IR remote produce the same effect as rotating the Dial in an anti-
clockwise or clockwise direction.

Up or down arrow keys simulate the CLICK function of the dial.

OPERATION

1.MENU KEY

Menu key is used to select the operating mode. The MODE is cycled as LIH -> LIVE -> PULSE ->
CONFIG and LIH again.
Present mode is displayed in Monitor1.

LIH (Last Image Hold)-Mode:

When the footswitch is pressed, X-Ray is turned ON and live real time image is displayed. When foot
switch is released, last image is frozen on the screen. LIH is the default mode on power up.

LIVE-Mode:

The live image is shown when X-ray is ON, but image is not captured when the footswitch is
released.
This mode can be used to compare the live image against an image captured earlier (on the LIH
Monitor)

PULSE-Mode:
In this mode X-ray is pulsed and images are captured in a sequence.

The Pulse Rate can be set by clicking Rotary dial. “LOW”, ”MEDIUM” or “HIGH” on monitor1.
(This rate also depends on the number of video frames averaged for noise reduction)

“LOW” PULSE rate causes the slowest operation (with longest X-ray off-time between successive
captures) and “HIGH” causes the fastest operation.
With “HIGH” rate selection, X-ray is kept continuously on and images are captured as fast as possible
with the current frame average setting.

This is to set the different configuration of the system such as File back up mode, IP address setting
for LAN, Date & Time setting and blanking circle position setting.

After editing all fields, the caption “IP Saved. Restart” will be displayed for 3 seconds (as shown
below). The unit has to be restarted to effect the changes by removing and re-applying power.

Important Note:
It is recommended to do this CIRCLE setting operation, at the time of installation only. (Changing the
circle causes circular window to be improperly displayed for earlier saved images.)
It is recommended to note down the old x, y & r-values, before attempting a change.
WE STRONGLY RECOMMEND THAT RADIUS BE KEPT AT MAXIMUM (139) AND
CAMERA ZOOMED TO COVER THE FULL CIRCLE SO THAT IMAGE DETAILS ARE
CLEARLY SEEN. FOR EXAMPLE, REDUCING THE RADIUS TO 125 HAS THE EFFECT OF
COMPRESSING THE IMAGE TO 90%.
Sub Menu: Delete Image: From CONFIG mode, select delete image & click encoder to enter this
sub menu. Display the image to be deleted on the second monitor & click rotary dial to delete the
image.

2. AVG Key

This key sets the number of video frames averaged for noise reduction (1, 2, 4, 8 or 16). Successive
depression of this key changes the value.( default 4)

3. ROTATE Key

This key enables the Image rotation operation. Rotation is possible on the LIH monitor only. The
LED above this key lit when this mode is selected.
The default rotation angle step is 20 degrees, clicking the encoder dial, changes the step angle to 2 (or
1 depending on model) degrees (and back to 20 degrees).

4. MIRROR Key

This is to view the Mirror view of the image displayed on the LIH monitor. The LED above this key
is lit when this mode is selected.

Mirroring is applied only on real time live images or images captured directly on the LIH monitor.

Mirror is not applied for any images transferred from Monitor-2 (by RECALL key ) or by READ
operation from USB / LAN.

5. NEG Key

This (NEGATIVE) key is used to show gray scale negative of images displayed on the monitors.
(Press the NEG key again to display the normal image).

6. CNTRST Key (Contrast Processing)

Image is captured with 256 levels (values 0-255) of grey scales (brightness levels). Contrast
processing operation involves stretching a portion of the Grey scale (say between 30 & 80) to full
brightness range so that the detailed information in this range is amplified and displayed.
This improves visibility of fine and hidden image details.

Press "Contrast" key to enter this mode, when following screen appears with a Contrast Slider.

The shaded region of SLIDE BAR shows the portion of the image being stretched / enhanced.(
extreme left side of the bar indicates image brightness of 0 ( black) and right most end indicates
image brightness of 255 ( white).

Steps for this operation is as follows,

1. Press "CONTRAST" key to enter this mode.


2. Rotate encoder (left / right) to move position of the shaded region.
3. CLICK encoder and rotate to change width of shaded region
4. CLICK encoder again to and rotate to change position of shaded region.( as in STEP 3.)
5. Repeat steps 3 & 4 to select region of interest / scan the grey scale information in the X-ray images.

Press the CONTRST key again to exit from interactive contrast enhancement mode.

7. SAVE key

This key is used to save the image on LIH Monitor to the current memory location on the Monitor-2
(image numbers 01 to 100 or REF image). If internal USB switch is ON then image also stored in
permanent memory, otherwise it will display error message “Permanent Storage Error”.

8. RECALL Key

The RECALL key is used to transfer the image displayed on Monitor-2 to LIH Monitor (applicable
only when X-ray is OFF / Foot switch is released).

This key can be used to selectively bring images from monitor-2 to LIH monitor and then to STORE
them to USB/LAN.

9. REF Key

REF image is a reserved location on the Monitor-2. Advantage of this reserved memory is that:
1.It can be displayed on Monitor-2 any time by pressing the REF key.
2. It is not over written in pulse mode of capture operation.

10. Zoom key

Press ZOOM key to activate ZOOM Mode. Two levels of zoom X2 and X3 are available,
which can be selected by repeated key press and it will be displayed at the right hand bottom
side of LIH monitor. The third key press exits from this mode.

Available only for LIH images.

In zoom mode the rotary dial is used to select the area of interest by horizontal and vertical
PAN. A click in rotary dial will switch between horizontal and vertical pan in this mode.
(Displayed as H Pan or V Pan)

Note: In ZOOM mode following operations will result in exit from ZOOM mode. -
Menu, Rotate, Mirror, Read, Store, Recall and Save.

11.READ and STORE Keys (Image store & Retrieval operations

Note: For this the File option in CONFIG. Sub menu should be = USB.

(Images in the pen-drive are sequentially numbered (3 digits FILE -No) with a Time stamp
(Yr.Mo.Dt. Hr.Min. Sec). This image number can be used as a reference number, to READ the
images from a PC or from the XC-V itself)

Storing images to USB – Pen drive:

1. Insert USB- pen drive into the front panel. (After inserting a drive wait for 15 seconds before
doing a read / store operation)
2. Press “ STORE” key.
3. Message “ STORE USB “ Is displayed, and image on the
LIH-Monitor is saved to next available sequential FILE Number.
(STORE key – LED also lights during this period).
Note: do not remove the pen drive during this period.

1. IP no: should be set properly in the XC-V CONFIG. – IP submenu.


2. PC - IP number should be set properly as in chapter E
3. The XC-V and PC should be on the same LAN network, physically connected via a LAN
SWITCH, or directly as given in chapter F.
4. The XC-V application software XC-IMAGER should be running on the PC.
5. Connect the PC to the XC-V unit, by giving the connect command (after opening a directory
on the PC with the required Patient ID).

LAN INTERFACE

Physical connection between X-Ray controller and a computer / computer network is by


means of a LAN cable. It is to be connected to the LAN connector provided at the rear side
of X-Ray controller. Please note that the type of cable will vary according to the LAN
connection as shown below.
X-ray controller can be connected to a computer in two ways as below.
a. X-Ray Controller directly to a single Computer.
b. X-Ray Controller to a computer or LAN network by using a hub / Switch /
Router.
For a dedicated LAN connection (X-Ray controller and a single computer) ‘cross wired’
cable should be used as shown in Fig. If it is to be connected by using a Hub / Switch /
Router a standard LAN cable can be used. Consult your PC supplier for more details.

7. Operation

7.1 Fluoroscopy

The C-arm has two Modes of operation

Normal Mode: In this mode, all fluoroscopic Parameters like KV & mA are to be selected
manually from the control Panel according to Patient’s Physique. The user can refer to the
Technic chart given in this manual for selection of the Parameters.

ABS Mode: (Automatic Brightness Stabilization Mode): This mode can be selected by
pressing the ABS Key on the Control Panel. A small LED lights up next to ABS key.
In this mode the machine automatically selects the fluoroscopic KV & mA depending upon
the Patient’s Physique. The thicker the part, the machine automatically selects higher
KV/mA for effective penetration & optimum Image quality.
The operating procedure of the C-Arm may also vary depending upon the configuration of
Image Memory, Monitors selected by the customer.
Appropriate section of the User Manual may be referred depending upon the requirement.

Before attempting to make an exposure, it is assumed that the following steps have already
been completed.
1. The system is connected to a proper supply source. C-Arm switched ON.
2. Memory/ Monitors & others options switched ON.
3. Patient is positioned on the C-Arm compatible Table.
4. C-Arm is positioned, aligned & height of C-Arm adjusted properly to expose area of
interest.
5. The F/R selector on C-Arm Control Panel to be selected at FLR mode.
6. The fluoroscopic parameters viz, KVP & FmA adjusted according to patient’s physique,
(Please refer to enclosed technique chart for optimum selection of parameters.)
7. Image Memory mode selector is at LIH & frames averaging selected between 2 to 16 for
best Image quality as desired.
8. The brightness & contrast of Monitors adjusted.
9. Necessary precaution for radiation safety to be taken by all persons in the operating
room.

Operation

7.1.1 Fluoroscopy on LIH (Last Image Hold) with Allengers-LM/VAM

Press the foot switch for 3-4 seconds & release it.
An Image would appear on 1st Monitor & would be held on the screen of the Monitor, Vary
the position/ height of the C-Arm to localize the area of interest in case Image taken on
monitor is not what is desired. When X-Ray exposure is ‘ON’ observe the reading on ‘FmA’
Display.
From the quality of the Image seen on the monitor, the operator can now decide if
fluoroscopic parameters viz. KVP needs to be readjusted according to the patient physique.
Now, press the footswitch again to take a new exposure of the desired part with properly set
exposure parameters. A new Image gets formed on the 1st Monitor & this Image gets held on
the monitor the moment exposure is released. The new image replaces the old image.
Press save switch on the memory unit. The Image gets transferred from the 1st monitor to the
2nd monitor at current memory location.
Pressing the foot switch at this stage would form a new image on the 1st monitor & gets held
the moment exposure is terminated. This new image can then be saved by pressing save
switch at a different memory locations selectable by UP/DOWN arrow or Rotary Dial on
memory.

IMPORTANT: Please note that every time the exposure is made the new image always gets
displayed only on the 1st Monitor. All live mode images also get formed only on the 1st
monitor.
The 2nd Monitor mainly displays the stored images & can be used as a reference image
monitor.
Continue with the operating procedure as described above till the time desired results are
obtained & surgical procedure is over.
The prints of various images can be taken on either Thermal Printer/ VCR/ Multi format
Camera etc., in case these optional devices are attached with the C-Arm.

7.1.2 Fluoroscopy on Pulse Mode


Selecting the Pulse mode on memory unit makes the unit to release the exposures in pulses &
not continuously. In this mode, pressing the foot switch continuously would release the
exposures to become ‘ON’ for a very short time & then becomes ‘OFF’ automatically. The
Image thus formed keeps on getting held on the 1st Monitor & the moment next pulse of
exposure comes, the old image vanishes. This way, the radiation to the operating staff &
patient is minimized. With the Menu Key/Rotary Dial, the Pulse rate can be changed as
Low, Medium & High.

7.1.3 Fluoroscopy on Live Mode


The live mode can be selected on the memory unit. In this mode, the image thus taken
cannot be memorized but appears on 1st monitor as long as the exposure is ‘ON’. The
moment the exposure is terminated the live image disappears from the monitor & the last
image held on the monitor appears.

Other functions on the image memory can be performed to obtain desired results. (Please
refer to earlier section of Image Memory Operations).

7.2 Radiography
The Allengers- HF – 46/49 C-Arm is set for an output of 30mA on Radiographic Mode. To
obtain desired ‘mAs’ in Radiography Mode, select the Radiographic time (in seconds) from S
+, - (Keys No. : 7) from 0.1 seconds to max. of 3 seconds.
(Example: If the time selected is 1.5 seconds & the respective LED indicating 1.5 seconds is
glowing the mAs output will be 30mA x 1.5 sec = 45 mAs).
Appropriate mAs values as indicated in the Radiographic Technique chart may be selected
for various Body Parts.

A universal cassette holder with Lead backing is supplied with the C-Arm unit.
Cassettes of size 8”x10” or 10”x12” can be accommodated in this cassette holder.
We recommend cassettes having intensifying screens of speed 400 or 800 may be used.

NOTE: Radiography can be performed on the C-Arm subject to availability of a suitable


power line having line resistance of 0.4 ohms as per IS:7620 (I) Table 3.

Satisfactory Radiographic operation cannot be performed on Power lines having line


resistance of more than 0.4 ohms (Ref. IS: 7620(I)).

The Allengers-HF – 46/49 C-Arm I.I.T.V. system is capable of performing Radiographic


functions. Following precautions must be taken before attempting to make a radiographic
exposure: -

a) Use only lead backed X-Ray Cassettes in the Universal cassette holder.
b) Never give a direct radiographic exposure without the X-Ray Cassette in place.

Proceed as given below to take an exposure:

1. Align C-Arm to patient.


2. Quickly assemble the universal cassette holder on the input screen of image
intensifier.
3. Slide the loaded X-Ray Cassette into cassette holder.
4. Plug in the 2 Pin Male connector of cassette holder to the socket provided at the
bottom of mounting cover near the I.I. tube.
5. On the control panel of C-Arm press F/R switch (3) to ‘R’ position.
6. Select desired Rad. Time as per the body part of patient with the help of seconds +, -
(Key 7). The selected time is indicated by respective LED.
7. Select desired KV on the control panel as per the penetration required for the body
part of the patient.
8. After all parameters are selected and patient/ C-Arm/ Cassette in position & aligned,
Press RAD. Exp. Switch (4) on control panel. The exposure is released & X-Ray
indicator (B) Glows.
9. Take out the cassette from the holder & develop the film in the dark room.
10. Remove cassette holder from the I.I. & keep at safe place.

7.3 Using C-Arm I.I.T.V. System


Some general important points to take care while using C-arm I.I.T.V.
1. Input voltage should be stable at 230 volts AC ± 2 volts Proper Earthing is must.
2. For Fluoroscopy, the Mode Selector switch should be at ‘F’ position & LED adjacent
to F must glow.
3. Whenever the image memory is made ‘ON’ the default parameter are set as under:
- Mode at LIH.
- Frames Integration at 4. (In Allengers LM/ VAM)
Keeping the frame averaging on Memory unit at minimum ‘0’, 1 (In Digital Image
Processor) or 2 in Allengers-LM/VAM may cause some snow to appear on the Image.
At this stage, No motion Artifacts can be seen. (This setting is more appropriate for
cardiac/ Uro studies & also where the part under observation is moving).
Keeping the frame Averaging at 4 & above may smoothen the Image with No Snow.
However the moving part may show blurring of the Image. At these settings the
Image may take more time to appear on the Monitor. This mode is preferable where
No movement of Part under observation is there (like in Orthopaedics).
4. The FmA display indicates the fluoro mA only during exposure. Otherwise it
displays 000.
5. In case the patient is closer to the X-Ray Tube Head, the image thus formed shall be
magnified. To overcome this problem the C-Arm height is to be adjusted in a way
that the patient is approximately in the center of the C-Arm. If the patient is closer to
I.I. Tube the magnification is minimum.
6. The C-Arm has an inbuilt Fluoro Timer of 300 seconds. After the lapse of 300
seconds of Fluoroscopy, further depressing the foot switch releases an audible alarm.
No image is thus formed, as X-Rays do not emit. To reset the fluoro timer, press
Reset switch (14) on the control panel. Depressing the foot switch now, will release
an exposure & fluoroscopy for further 300 seconds can be performed.

Maintenance

10. Maintenance

10.1 Overview
Regular Maintenance of the Allengers HF-46/49 C-Arm System is necessary for safe and
trouble free use. Periodic maintenance helps in reducing equipment downtime, service cost
and operational hazards.
Contact the local service representatives for parts or planned maintenance inspections. It is
recommended that maintenance be performed on the system every three months.

10.2 Inspecting the System


Daily checklist.
• After each use, remove the Power Cord from the Power outlet and cover the machine
properly.
• Clean the Tube Head and exterior appearance of the unit by dry cloth.

Weekly checklist
• Check system power cord for any cracks or damages.
• Check all the connectors outside the machine for their firmness
• Clean the foot switch with a soft dry cloth.

Monthly checklist
• Visually inspect the unit every month.
• Check for mechanical problems or keyboard problems.
• Check the electrical and power cables for cracks, cuts.
• Check the equipment for loose or missing hardware.
• Check for any leakage from the Tube Head
• Check for any abnormal noise or sparking in the machine.

Maintenance

10.3 General Maintenance


1. Since only trained & qualified personnel should be permitted access to the internal
portion of the X-Ray unit. It is recommended that Service and Maintenance be performed
by our local ALLENGERS Service Engineer.

2. User should confine only to preserve the exterior appearance of the unit by cleaning the
unit with dry cloth. It is recommended to take suitable precautions against accumulation
of dust etc. on the Control Panel, Tube Head, and Fluoroscopy etc. Dust Covers may be
used.

3. ALLENGERS MEDICAL SYSTEMS LTD. maintains a network of Service Centres


from which the assistance of skilled service personnel may be obtained. You are
requested to contact any of the centre near you for all your service related problems.
Telephone no. of our Service Centre are mentioned in this Manual.

4. Tube Current Adjustments:


Following the installation, occasional readjustment of Tube current will be required
because of changes in electron emission of the filament of X-Ray Tubes brought about by
evaporation of Tungsten.

When the mA values are more than 10% off from the proper values, the tube current
must be readjusted. This should be done by qualified Service personnel specially trained
in servicing the equipment.
