I, DIOSDADO M. MARCAYDA, of legal age, married to ARLENE D. MARCAYDA, FILIPINO, and a

resident of 128 Wakas Mambog 4, Bacoor, Cavite 4102, Philippines, having been duly sworn in
accordance with law, hereby depose and state:

1. That my name is DIOSDADO M. MARCAYDA and my place of birth is in PARANAQUE,

METRO MANILA as stated and registered in my Birth Certificate issued by Office of the
Civil Registry od Paranaque City.

2. That since childhood, I have been using the name DIOSDADO M. MARCAYDA in my
personal records, transaction and communications;

3. That on my Marriage Certificate, my Name appears as DIOSDADO M. MARCAYDA with

Place of Birth in CAMALIG, ALBAY;

4. That the Names DIOSDAO M. MARCAYDA with Place of Birth in PARAÑAQUE, METRO
MANILA and DIOSDADO M. MARCAYDA with Place of Birth in CAMALIG, ALBAY refer to
as one and the same person;

5. THAT I am executing this Affidavit to attest to the truth of the foregoing facts and to use
the same for whatever legal purpose it may serve.
