Research Instrument

The primary tool or instrument used for this research is a self-administered


questionnaire to obtain data. The study will use a self-administered
questionnaire to obtain data. According to Kothari (2004), questionnaires
provide significant merit in administration by providing an even stimulus
potentiality to a vast number of individuals at the same time and allowing the
inquiry to accumulate data quickly. The researchers chose self-administered
questionnaires based on these advantages.

A self-administered questionnaire using open-ended questions and


close-ended questions was prepared to gather data pertaining to the factors
that influence teenage pregnancy and what impact does this have on them. On
a five-point scale ranging from “1” to 5,” the adolescents will be rated on how
big the impact of teenage pregnancy is on them. The questionnaires will be
conducted with the local dialect, Cebuano or English, in order to facilitate the
questionnaire process among respondents who prefer the local language for
the self-administered questionnaires. The questionnaire will be a way for the
researchers to procure knowledge and information on the topic.

This questionnaire is part of our academic research on the subject APP 006. We,
the students from the 12th grade in I4 STEM Health, Southwestern University
PHINMA sympathize with your busy schedule and ask if you could spare some
minutes to fill out this questionnaire. The study is about the role of parenting
styles in the academic performance of students of selected respondents from
Southwestern University PHINMA.

For your knowledge, this survey intends to better understand the factors that
influence teenage pregnancy among chosen Filipino adolescents from North
Poblacion, City of Naga, Philippines, and the impacts of teenage pregnancy on
young teenagers in their lives. Read each question carefully and answer it as best
