1. INTRODUCTION
1.1 OBJECTIVES OF THE DOCUMENT
Purpose of this document is to define the minimum Health, Safety and Environmental (HSE) requirements to
be followed and applied by VENDOR and SUBCONTRACTOR (herein after SUBCON) during the execution
of their scope of work within PT. Saipem Indonesia Karimun Yard premises.
This document collects the main HSE requirements for SUBCON, while details are addressed within
Company HSE Management System (specific procedure/specification to provide upon request). This shall
not release SUBCON from its obligation to comply with the HSE requirements established within the contract
provisions.
Remains as obligation for SUBCON to ensure that adequate safety levels are maintained and environmental
impacts are monitored throughout the PROJECT execution.

1.2 AREA OF APPLICATION


This Specification applies to PT. Saipem Indonesia Karimun Yard.

2. DEFINITION, ABBREVIATIONS AND ACRONYMS


2.1 DEFINITION
Company PT. Saipem Indonesia

SUBCON General abbreviation used to identify either Vendor and/or Subcontractor working
within Karimun Yard.

Subcontractor General term to define other companies providing services Saipem within
Karimun Yard on the basis of a contract, where Company has the responsibility to
set the HSE standards or to ensure that the Subcontractor’s HSE Management
System is in line with the Company’s HSE Management System.

Vendor General term to define the vendors of good and/or package on the basis of a
contract, where Company doesn’t have the responsibility to set the HSE
standards, nor directly supervising the activities. The Vendor applies its own HSE
Management System. Whenever the Vendor requires to perform part of the
activities foreseen by the contract within Karimun Yard, it shall be considered as
Subcontractor.

3. HEALTH SAFETY AND ENVIRONMENTAL REQUIREMENTS


3.1 HSE GENERAL REQUIREMENTS
The general HSE requirements that SUBCON must comply with are the following, but not limited to:
a) All SUBCON’s personnel shall comply with Saipem Karimun Yard HSE Management System
provisions.
b) Saipem reserves the right to conduct HSE audits on SUBCON’s working procedures and equipment at
any time, upon previous notification given sufficiently in advance.
c) The SUBCON is contractually obliged to observe and comply with all currents applicable legislative
requirements and guidance concerning the HSE and Welfare of personnel. All documents required by
law shall be at site and made available to Control Authorities and Saipem.
d) When mobilized to Karimun Site, the SUBCON’s employees shall be always in possession of a valid
Medical Fitness Certificate. Saipem Medical personnel will have free access to the Medical Fitness
Certificates, which shall be easily available on site, always taking into account the confidentiality of the
private medical data.
e) If SUBCON disposes of its own facilities within Karimun Yard, it shall develop a MEDEVAC Plan.
Saipem will organize, the Emergency Medical Evacuation (MEDEVAC) of SUBCON’s personnel up to
the nearest reliable hospital (Primary evacuation). Further treatment and transfer of the patient are
SUBCONs’ responsibility.
f) The SUBCON shall properly plan and organize own activities and works; all the existing risks shall be
assessed (e.g. interference risks, electrical hazards, mechanical hazards, risks of falling from heights,
risk burial, exposure to physical, chemical, radioactive sources).
g) The SUBCON shall provide to its personnel all equipment and devices required for carrying out the work
as well as all the necessary Personal Protective Equipment (PPE).
h) All SUBCON's own equipment used within Karimun Yard shall be tested, certified and compliant with
the applicable legislation.
i) The use of any chemical substance shall be preliminarily approved by Saipem representative. Prior
starting any work activity, the SUBCON shall submit the written procedures for any services involving
the handling or use of hazardous or toxic substances.
j) The SUBCON shall comply with the Saipem Environmental Management Plan and all applicable
environmental laws.

k) The SUBCON shall immediately report every incident involving its personnel and/or equipment to
Saipem Representative. Then, the official event notification shall be issued not later than 24 hours after
the incident. The investigation shall be then carried out involving Saipem personnel.
l) The SUBCON working within Karimun Yard must attend Company induction program, which includes
instructions given to each new employee, regardless of prior experience.
m) SUBCON employees have to be trained to those subjects pertaining to local realities / threats / HSE risk
factors, in compliance with their professional position, and scheduled so as to provide a sufficient
number of First Aid trained personnel continuously available on site.

4. REQUIRED DOCUMENTS
4.1 COMPANY HSE REQUIREMENTS DOCUMENTS
The SUBCON is required to comply with the relevant HSE requirements documents submitted along to this
Specification, within the Scope of Work package. Here below the summary of Company HSE Specifications
which might be sent to the SUBCON based on its Scope of Work:
• HSE Requirements for Subcontractors – Low HSE Risk (Doc. No. SPC-COR-HSE-001-E)
• HSE Requirements for Subcontractors – High HSE Risk (Doc. No. SPC-COR-HSE-002-E)
• HSE Requirements for NDT Radiographic Activities (Doc. No. SPC-COR-HSE-006-E)
• HSE Requirements for Subcontractors for Fabrication Works (Doc. No. SPC-COR-HSE-007-E)
• Project HSE Requirements documents (Project based document list)
As per Company Work Instruction ‘HSE in Procurement’ (Doc. No. WI-PTSI-HSE-025-E), SUBCON
proposals shall be assessed during tendering stage, by compiling the HSE Bid Evaluation FORM (Doc. No.
FORM-PTSI-HSE-028-E).

4.2 HSE DOCUMENT T O BE PROVIDED DURING BIDDING STAGE


4.2.1 HSE Governance
a) Copy of Certification (OHSAS 18001, ISO 14001, etc.), if any; plus the HSE MS Manual.
b) Project Organization Chart, specifying the number and the appointment of HSE personnel.

4.2.2 Health and Safety Management


a) Proposed Project HSE Plan addressing the following, but not limited to:
• HSE Organization and Responsibilities;
• Hazard Identification and Risk Assessment including the Health Risk Assessment;
• Emergency flowchart (bridging document with SIKB Emergency Procedure);
• List of HSE Procedures applicable to the Vendor or Subcontractor scope of work.
b) Protocol for periodical medical fitness examination.
c) Description of provision and upkeep of Personal Protective Equipment (PPE), both standard issue and
required for specialized activities.
d) Description of management of plant and equipment used within SUBCON’s premises, sites or other
locations with reference to registration, control and maintenance in a safe working condition.

4.2.3 Health and Safety Past Performance


List of the last five years safety statistics within SUBCON’s organization.

4.3 HSE DOCUMENT T O BE PROVIDED UPON CONTRACT AWARD


The SUBCON is required to provide at least 15 (fifteen) days prior mobilizing its personnel at Karimun Yard,
the following information and documentation:
a) Medical Fitness Certificates of all personnel intended to be mobilized at Karimun Yard;
b) Project HSE Plan (plus any other HSE procedure) for the activities subcontracted;
c) Detail Scope of Work and Method Statement;
d) Risk Assessment for the subcontracted scope of work;
e) Copy of Personnel Certification & Training;
f) Waste Management Plan;
g) Copy of valid Equipment Certificates;
h) Material and Substance Information / MSDS (Material Safety Data Sheet).
Detailed list of documents is hereby attached in Appendix A.

5. SPECIFIC HSE REQUIREMENTS DURING CONTRACT EXECUTION PHASE


5.1 LEADERSHIP
SUBCONs shall encourage all members of the workforce to monitor the unsafe acts or unsafe conditions
throughout the course of the work. Where an employee identifies or observes an unsafe act or situation,
work shall be stopped, reported and/or remedied as appropriate. It is everyone’s right and responsibility to
intervene (i.e. STOP Policy) when safety is being compromised in order to be safe, look after the safety and
well-being of others.
SUBCON, upon specific Company request, shall implement Company Behavioural Based Safety Program
called Leadership in Health and Safety (LiHS) Program.

5.2 HSE ORGANISATION


SUBCONs shall appoint the following qualified personnel for each worksite:

5.3 ENTRY REQUIREMENT TO CONTRACTOR PREMISES


SUBCON shall comply the Karimun Yard Entry HSE Requirements (Appendix A). Checklist to be provided
to Saipem for its evaluation prior SUBCON mobilization.
Upon SUBCON personnel mobilisation at Karimun Yard, a complete HSE induction shall be attended. In
addition, specific training such as PTW and JSA issuance shall be attended.
Equipment and tools inspection, as well as PPE inspection shall be performed by Saipem HSE personnel,
prior accessing Karimun Yard working areas. PPE to be in compliance with the Standards mentioned in
Appendix A, Section PPE.
SUBCON shall not be allowed to enter Karimun Yard unless all required items on the Appendix A are fully
fulfilled.

5.4 COMMENCEMENT OF THE WORK


SUBCON shall ensure that the Specific HSE Risk Assessment covering subcontracted activities shall be
completed and approved by Company and no outstanding actions prior to perform the activities.
SUBCON shall ensure that training certificates of its personnel are made available to the Company upon
request.
SUBCON shall provide their personnel and visitors with all items of equipment and clothing necessary for the
safe and healthy conduct of the job; training/induction sessions on their use shall be conducted and
competence/awareness assessed.
