I, Marilou Nicandro, the undersigned, of legal age and a resident of Pinagbayanan Pila, Laguna ,
hereby authorize the Dean of the College of Business, Administration and Accountancy (CBAA) of
Laguna State Polytechnic University to let my son/daughter J nnylyn Nicandro to participate in the
academic activity entitled “HIRAYA MANAWARI” of Junior Philippine Institute of Accountants (JPIA).

I understand that it is a monthly session and the first set will start with the topic “Introduction
and Completion of Accounting Practice Set (ICAPS)” starting on January 16-19, 2023 from 9am to
4pm. This aims to build and strengthen Bachelor of Science in Accountancy students' knowledge
about the course and is designed for students who are having difficulty answering problems at their
own pace. The said activity will be conducted face-to-face, headed by Mr. Edison A Tamayo, CPA
and JPIA officers and will be held at CBAA Rooms.
