1. PURPOSE
The process of work permit obtaining ensures that foreseeable hazards and their consequences are
identified, actions are taken to prevent them, and appropriate controls are in place. All workers perform-
ing works under work permits must be familiarized with the Procedure, Work Execution Plan (hereinafter
referred to as WEP), Risk Assessment (hereinafter referred to as RA) and Work Safety Analysis (herein-
after referred to as WSA) for the relevant type of works. The purpose of the work permit system is to en -
sure proper control and mitigation of risks with regard to a certain type of works or several types of works
performed at the site, including simultaneous operations. Work permits and appendices thereto are regu-
lated documents that authorize a certain number of responsible persons to perform certain works in a cer-
tain area at a certain time and contain detailed safety measures aimed at safe performance of a particular
task.
This Procedure describes the order of issue of work permits (WP) for performance of works at the
Site developed by the Contractor, which is used to ensure safe performance of works and contains pre-
ventive measures to be taken before and during work performance.
The purpose of obtaining a work permit is to determine requirements and measures relating to is-
sues of occupational health, fire and industrial safety and environmental protection (hereinafter referred to
as OHFISE) to perform certain types of works after assessing the hazards, working environment and
risks.

2. SCOPE OF APPLICATION
This procedure on arranging activities under work permit (hereinafter referred to as the Procedure)
applies to all works performed at the NPP construction site (hereinafter referred to as the Site). The Pro-
cedure applies to the personnel of the branch of JSC CONCERN TITAN-2 (hereinafter referred to as the
Contractor), Sub-contractors, and their sub-contractors performing works at the Site.
The work permit system is an essential requirement for ensuring safe working conditions and con-
stitutes a part of the safe work practices used to control works defined as potentially hazardous. The Pro-
cedure provides for identification of hazard factors relating to the work to be performed, assessment of
risks associated thereto, provision of work controls, management of these controls, and monitoring of the
work to ensure safety of workers and the environment.
The Procedure defines the following stages of work execution under a work permit:
‒ determination of the scope of works to be performed;
‒ defining of hazards;
‒ defining and description of safety measures;
‒ drawing up of a work permit;
‒ performance of works;
‒ completion of works under the work permit.
Note: All Contractor’s and Sub-contractor’s employees must remember that the work permit sys-
tem alone is not an absolute guarantee of safety. Each employee is responsible for compliance with safety
requirements in the course of work performance. The mere fact of issuing a work permit does not make
the work safe. This can be achieved through implementation of organizational and technical measures be-
fore commencement of works.
Works with consistent conditions (in particular: location, risks, equipment, the team performing
particular types of works) may not need a work permit; for this purpose, a list of processes is agreed with
the OHFISE Department and approved by the Program Director. For such works, risk assessment must be
conducted at least once a month.

Terms Definitions
Hot works supervisor an employee assigned to monitor the performance of hot works in
areas with complex architectural, planning and/or process solutions
(two or more vertical structural levels (floors, tiers), two or more hori-
zontal rooms with process-oriented openings), allocated from among
the team or separately. Personnel assigned as supervisors must be
properly trained in fire safety measures when performing hot works
and activating automatic fire protection systems in remote (manual)
mode, as well as the use of primary fire extinguishing equipment.
Confined/enclosed space an employee assigned to supervise works in CES. The supervisor must
works (CES) supervisor be properly trained to perform his or her duties. The supervisor is ap-
pointed from among the workers for each WP separately. He/she must
remain outside CES for as long as works in the CES are in progress.
Work permit a written order for safe work performance, defining the scope, place,
time and conditions of its execution, as well as the necessary safety
measures, composition of the team and persons responsible for the
safety of works.
Responsible work supervisor an employee of the Sub-contractor from among the engineering per-
of the Sub-contractor sonnel who is responsible for applying the work permit system with
regard to works on the corresponding site. Sub-contractors shall
identify and designate their site and work area supervisors and provide
the Contractor’s Safe Working Practice Manager with a list of super-
visors. The same person cannot combine the roles of the Responsible
Work Supervisor and Work Responsible Person within the same WP.

Extremely hazardous works works performed in areas of constant or possible exposure to occupa-
tional hazards, the occurrence of which is not related to the nature of
the works performed, which requires additional safety measures to be
developed and implemented for each specific production operation
prior to commencement of such works.
Authorized electrical person- qualified personnel with appropriate training and certificates, author-
nel ized to work with electric plants, as well as to carry out measuring and
control works at electric plants. Involved in the performance of works
under WP in the case of works with electric plants, in protected areas
of power lines, as well as in the presence of the risk of electric shock.
Emergency situation (ES) current state of the service facility resulting from an accident, natural
hazard, natural disaster, natural or other calamity that may or has
already led to the death of people, damage to health or the environ-
ment, significant loss of property or disruption of human life.

5. RESPONSIBILITY
Program Director shall be responsible for ensuring that organizational, technical, human and fi-
nancial resources are available for the Procedure implementation.
Chief specialists of the Contractor’s Inspection Control Division shall be responsible for:
‒ monitoring of compliance with the requirements of the Procedure, as well as safety requirements
specified in WP on the territory of the Site;
‒ participation in Risk Assessment and verification of the availability of qualification certificates and
required training of personnel;
‒ checking risk assessment results, completeness of the developed measures in the preparation for and
performance of works;
‒ stoppage of works in case of identification of violations of safety requirements that put workers at
risk, and assisting in arrangement of corrective actions.
Radiation protection specialist of the Contractor shall be responsible for:
‒ ensuring the most effective management of activities related to the use of IRS by reducing, to the ex-
tent possible, influence of IRS on exposed personnel;
‒ control and monitoring of the movement of all ionizing radiation sources on the Site;
‒ control of RS documentation before commencement of works with IRS;
‒ monitoring of availability of a competent supervisor of works with IRS, appointed accordingly;
‒ monitoring of availability of and compliance with the procedure for IRS safe operation, as well as
emergency response procedures;
‒ verification of appropriate training of personnel and availability of equipment for IRS operation;
‒ arrangement of periodic training sessions;
‒ checking the arrangement of access control to the area where works with IRS are carried out;
‒ verification of correct calibration and periodic testing of radiation level measuring tools;
‒ ensuring that registration documents relating to works with IRS are up-to-date;
‒ examination of exposed personnel and arrangement of monitoring of those working with IRS;
‒ control of the level of personnel exposure, ensuring that permissible doses are not exceeded;
‒ verification of comprehensive medical examination of all workers exposed to ionizing radiation and
control over the maintenance of relevant medical records.
WP coordinator of the Contractor shall be responsible for:
‒ coordination of the work permit issue process and assisting the Sub-contractor with the work permit
system;
‒ registration of work permits in the unified registry, assigning WP numbers;
‒ development of training on the WP system and arrangement of training of responsible managers of
structural subdivisions of the Contractor and RWS of Sub-contractors;
‒ updating of training in case of changes in the Procedure;
‒ keeping of copies or scanned copies of WP;
‒ arrangement of audits of the WP system of Sub-contractors.
WP coordinator of the Sub-contractor shall be responsible for:
‒ coordination of the process of obtaining work permits for employees of their organization;
‒ checking new work permits filled out by the WRP (work permits are filled out in two languages: Eng-
lish and the language understood by the holder of the work permit (responsible work performer) and
signed by the responsible persons);
‒ providing work permits to the WP coordinator of the Contractor for registration and number assign-
ing;
‒ checking the availability of daily renewal of work permits in the work area;

‒ notifying the WP coordinator of the Contractor on a daily basis (sending updated information on the
status of open and suspended work permits via e-mail);
‒ ensuring availability of open work permits at the work site and archiving of closed work permits;
‒ arrangement, jointly with the WRP, of monitoring of the full range of preparatory works in accor-
dance with the description in WP;
‒ arrangement, jointly with the WRP, of monitoring of the availability and serviceability of the neces-
sary fire and safety equipment in the work area;
‒ checking the availability of a full package of necessary documents for opening a work permit (e.g., an
agreed and approved WEP for work performance, underground utilities diagram, rescue plan, etc.);
‒ training of employees of their organization to work in accordance with the requirements of the Proce-
dure on work permits;
‒ keeping records of employees trained in accordance with the requirements of the Procedure.
Head of OHFISE Department shall be responsible for:
‒ arranging for preparation and enactment of the Procedure;
‒ arranging for control over compliance with requirements of the Procedure at the Site;
‒ arranging for familiarization of Sub-contractors’ employees with the requirements of the Procedure.
Hot works supervisor shall be responsible for:
‒ visual control of the areas of hot works, adjacent rooms vertically and/or horizontally at a distance
sufficient to take emergency measures to prevent an outbreak and spread of fire (ignition) in the area
of performance of hot works and fire hazardous works, as well as in adjacent rooms and at adjacent
process installations and equipment;
‒ timely warning of the personnel performing the work in case of an outbreak of fire (ignition), changes
in the situation associated with the emergence of danger to life and health as a result of impact of an
abnormal hazard;
‒ inspection of rooms, lower floors (tiers) adjacent to the area of performance of hot works for the ab-
sence of combustible substances and materials, explosive and flammable concentrations of steam-air
medium, both before and during the works;
‒ inspection of rooms, lower floors (tiers) adjacent to the area of performance of hot works, as well as
adjacent process installations and equipment, for the ingress of red-hot metal particles, sparks, spray
from grinders, etc.;
‒ timely notification of the stoppage of hot works in case of a threat to the life and health of workers as
a result of a fire (ignition) or other emergency situation, as well as in case of activation of automatic
fire protection systems;
‒ provision for free approaches to the workplace and escape routes in the area of works;
‒ ensuring the fulfilment of their duties for the entire period of hot works at the assigned place of their
performance;
‒ timely use of primary fire extinguishing equipment in case of a fire (ignition).
Confined/enclosed space works supervisor shall be responsible for:
‒ conducting measurements of the air in CES;
‒ monitoring of the number of employees in CES, keeping a book of CES entry/exit;
‒ marking of safety ropes so that each worker in CES can be easily identified in the event of an emer-
gency situation (in the case of more than one worker in CES);
‒ periodic check of well-being of workers, as well as evacuation of the workers in case of ES;
‒ monitoring of conditions inside and outside CES before entering and in the process of work;
‒ maintaining communication with workers inside CES;
‒ immediate evacuation of personnel from CES in the following cases:
‒ detection of unacceptable conditions;
‒ occurrence of exposure to hazards;

‒ conducting briefing on a shift basis with those performing works under work permit with regard to the
safety of works and activities before commencement of works;
‒ providing information about the upcoming work to all work performers, in particular as related to the
scope of works, safety requirements in the course of work, potential hazards, restrictions and condi-
tions of work, required equipment and PPE;
‒ verification of the training and qualifications of personnel who are assigned to certain duties (such as
the person responsible for air measurements in enclosed spaces, the lookout observer, the enclosed
space safety guard, personnel operating HMM, authorized electrical personnel, etc.);
‒ submission of applications for planned works at least 24 hours before their commencement (by hand
or by e-mail) to the WP coordinator of the Contractor;
‒ placement of WP in the work area;
‒ extension of WP in the work area on a shift basis, as well as in the case of extending the hours of the
work shift to complete a continuous work process (e.g. concrete pouring, welding, load handling oper-
ations), WP validity must be extended by the RWS’s signature in a corresponding box in the WP
form;
‒ notifying immediate WP coordinator of work permits under which the works will be performed and
which ones will be suspended for the current shift;
‒ presence at the work site to monitor the compliance with the requirements specified in WP;
‒ stoppage of works in case of detection of unsafe actions or conditions, and taking corrective mea-
sures;
‒ ensuring proper handover of the workplace at the end of a work shift;
‒ ensuring the safety and cleanliness of the work area upon completion of works;
‒ transfer of his/her duties to the DWRP in case it is necessary for him/her to leave the work site.
‒ Isolation/interlock responsible person shall be responsible for:
‒ correctness and completeness of the determination of isolation points;
‒ availability of the necessary disabling devices (locks, chains, plugs, etc.), as well as warning signs and
inscriptions;
‒ ensuring reliable isolation, testing, dismantling for testing, and relocation of equipment in case of a
long-term isolation process;
‒ periodic monitoring of the installed isolation devices;
‒ interaction with WRP;
‒ keeping an interlock/isolation log;
‒ storage of interlocking devices (locks, keys, tags, chains, blockers, clamps, etc.).
Site employees, not related to engineering personnel, shall be responsible for:
‒ listening to briefings, trainings on compliance with the requirements of the Procedure;
‒ compliance with the requirements of the Procedure when performing their duties at the Site;
‒ notification of superiors on cases of failure to comply with the requirements of the Procedure.
‒ Managers of subcontracting organizations shall be responsible for:
‒ allocation of a WP coordinator in their organizations;
‒ communication of the Procedure requirements to all employees of the organization;
‒ arrangement of risk assessment procedure;
‒ appointment of responsible work supervisors responsible for compliance with the requirements of the
Procedure, by area and type of works;
‒ compliance with the Procedure requirements within their organizations.
Authorized electrical personnel shall be responsible for:
‒ determining the presence of voltage on lines and equipment;
‒ placement of appropriate lockout/tagout devices, placement of electrical safety signs;
‒ control of electrical safety in work areas.

Excavation works on depths more than 1.2 m are considered to be CES works in the following sit-
uations:
‒ the depth of excavation is greater than the width;
‒ it is difficult to access the area of works in the excavation;
‒ the excavation contains or has the potential to contain hazardous gases.
In the case of classification of works in the excavation as CES works, it is necessary to perform
the works under CES WP.
Works performed under WP include:
‒ drilling works;
‒ piling;
‒ excavation works (soil excavation, backfilling, mechanized grading, tamping);
‒ road maintenance;
‒ paving and tile setting;
‒ slope stabilization.
WP for excavation works shall contain the following information:
‒ permission to carry out excavation works in accordance with the specified safety requirements;
‒ permission to work for those responsible for the technical integrity of underground networks and utili-
ties, subject to special safety measures;
‒ RWS approval;
‒ obligation of WRP to take into account and comply with the required safety measures in the prepara-
tion and conduct of excavation works, as specified in WP.
WRP shall conduct a daily briefing with those performing works under WP with regard to the
safety of works and activities before commencement of works.
WP for excavation works shall be drawn up in accordance with the form specified in Appendix 1
with the check-list given in Appendix 2 attached.
6.2 Work Permit for Electrical Works
Responsible employees must evaluate hazards / risks before working on and/or near electrical
equipment and, if present, the risk of electric shock, and determine the necessary safety measures and
controls required to perform the works.
Exposed live parts of equipment that are inside or near the area of works performed under WP
must be disconnected and secured in a safe position by lockout/tagout means (LOTO).
Work Responsible Person shall conduct a daily briefing with those performing works under WP
with regard to the safety of works and activities before commencement of works.
Areas of work with electrical equipment must be fenced, marked with signs; access by unautho-
rized persons to these areas must be restricted.
WP for electrical works is issued only to qualified electrical personnel authorized to work with
electric plants.
WP for electrical works shall be drawn up in accordance with the form specified in Appendix 1
with the check-list given in Appendix 3 attached.
6.3 Work Permit for Civil Works
WP for civil works shall also include works that do not create a fire hazard (no spark formation,
creation of high temperatures, open flames capable of igniting flammable materials, HI&CL, fuels and lu-
bricants, etc.), and issued if the planned work is not attributed to any of the established types of WP.

Types of works with the issue of WP for civil works include the following:
‒ sandblasting, structure corrosion removal without spark formation;
‒ preparation for painting, painting, drying of painted surfaces;
‒ concreting works;
‒ reinforcement cage tying;
‒ installation/dismantling of frameworks and plasterboard cladding;
‒ survey works;
‒ insulation operations;
‒ testing of pipelines and pneumatic pressurised systems.
Work Responsible Person shall conduct a daily briefing with those performing works under work
permit with regard to the safety of works and activities before commencement of works.
WP shall be drawn up in accordance with the form specified in Appendix 1 with the check-list
given in Appendix 4 attached.
6.4 Work Permit for Hot Works
Work permit for hot works is issued when organizing and performing one or more types of works
at temporary locations using an open flame or other source of ignition.
Hot works shall include:
‒ fire heating of bitumen/surfaces;
‒ gas and electric welding works;
‒ gas and electric cutting works;
‒ gasoline and kerosene cutting works;
‒ soldering works;
‒ metal cutting using a mechanized tool.
This type of work permit is issued when performing hot works with electrical, electronic, gas-
welding, gas-cutting, gas-flame equipment, electrified tools for soldering works, electro-mechanized tools
for cutting (grinding) metal, as well as gasoline and kerosene lamps and other devices that create an open
flame, red-hot metal melts and sparks with high heat generation, which can become a source of fire (igni -
tion).
Work Responsible Person shall conduct a daily briefing with those performing works under work
permit with regard to fire safety and safety in other areas before commencement of works.
At the end of each shift or during a long break in work, all ignition sources must be shut off and
any machinery or equipment shut down.
WP is issued for a period of 2 weeks under the following conditions:
‒ only works specified in WP are performed;
‒ there are no changes in working conditions (same personnel, same works, same work environment,
etc.) after the initial approval and issuance of WP.
If testing of the environment for gassiness is required, WRP must ensure that the area is tested
with a gas analyzer (duly calibrated) and that the LEL (%) is determined before commencement of works,
as well as during the works, with the frequency specified in WP. The serial number of the gas analyzer
used and the measurements taken should be recorded in WP.
WRP shall conduct briefings on a shift basis with regard to the safety of hot works prior to com-
mencement of the works.
The area of hot works must be free of combustible materials, gas cylinders, flammable and com-
bustible liquids, combustible lubricants, explosive materials, etc.

In the absence of technical possibility to remove combustible materials (but not gas cylinders,
flammable and combustible liquids, combustible lubricants, explosive materials), the area of hot works is
isolated with a firefighting blanket, and an observer is placed in the work area with primary fire extin-
guishing equipment.
If it is necessary for the observer to leave, his/her replacement must be arranged; otherwise, hot
works are to be suspended for the period of his/her absence.
Hot works must be stopped immediately if the conditions of the works have changed. In the event
of work stoppage due to LEL exceeding, WRP must be notified immediately.
In the case of hot works at height, it is necessary to ensure that sparks and molten droplets do not
fall on the below areas, structures and equipment. The area beneath the hot work area must be fenced off;
it must be ensured that there are no workers in it, and a signalman is to be appointed.
WP for hot works shall be drawn up in accordance with the form specified in Appendix 1 with the
check-list given in Appendix 5 attached.
6.5 Work Permit for Works at Height
Works at height include works at any place where, if proper precautions are not taken, a person
could fall from a height sufficient to cause bodily injury. If there are risks associated with the possible fall
of a worker from a height, WP is to be obtained.
Works at height include the following:
‒ installation / dismantling of scaffolding;
‒ works at height without the use of scaffolding and trestlework, with the use of rope access systems;
‒ works with the use of construction elevators;
‒ works on scaffolding and trestlework;
‒ works on a flat roof;
‒ works at ground level in places adjacent to deep excavations;
‒ works on the installation of formwork inside deep excavations.
Works at height are allowed only if there are no alternative options for the performance of works
without the risk of falling (assembly of structures, the use of machines, etc.). Works at height are permit-
ted only when safety ties with double shock absorber are used.
Work Responsible Person shall conduct a daily briefing with those performing works under work
permit with regard to the safety of works and activities before commencement of works.
WP for works at height shall be drawn up in accordance with the form specified in Appendix 1
with the check-list given in Appendix 6 attached.
6.6 Work Permit for Works with IRS
WP for works with the use of ionizing radiation sources is issued for any works with IRS at the
Site.
Transportation, storage and use of IRS must be carried out in accordance with the requirements of
the on RS.
WP is issued following a coordination meeting on the WP in the presence of the head of the Con-
tractor’s structural subdivision (or his/her representative), the Contractor’s radiation safety officer, and
the Contractor’s WP coordinator to ensure compliance with the requirements on RS.
Radiation safety officer of the Sub-contractor shall specify the type of IRS, its identification num-
ber, and decay curve, as well as record the time of movement of IRS from the storage facility to the work
area and back in the IRS movement log.

Works with the use of IRS shall be carried out during the periods of the lowest concentration of
workers at the site (at night, on weekends and public holidays); approval and signing of WP shall be com-
pleted well in advance during the daytime, after which all participants of construction shall be notified in
advance of the upcoming works with IRS.
The application for work with IRS shall be sent to the Contractor’s WP coordinator and radiation
safety officer at least 24 hours in advance. Before commencement of works, the radiation safety officer
shall make sure that there are no unauthorized persons in the work area and that there are no fences or RS
signs restricting access.
The radiation safety officer must be present in the area where operations with IRS are carried out
to monitor compliance with radiation safety measures and to sign relevant documents.
WP for works with IRS shall be issued for one shift (no more than 11 hours). For each IRS, a sep-
arate WP is to be issued.
WP shall be drawn up in accordance with the form specified in Appendix 1 with the materials
given in Appendix 7 attached.

mantling of Fences
Any works on dismantling of structures covering process-oriented opening (shields, decks,
hatches, etc.), as well as on dismantling of any other collective protective equipment must be carried out
with obtaining an appropriate WP. WP must be accompanied by a diagram showing the relevant areas of
work and places of removal of fences and decking, as well as alternative routes of access and evacuation.
This WP is attached to the WP for works (WP for CES works, WP for hot works, etc.) and com -
prises an integral part thereof.
WP shall be drawn up in accordance with the form specified in Appendix 1 with the check-list
given in Appendix 8 attached.
6.8 Work Permit for Hoisting Works
WP shall be issued for all types of hoisting works.
Before obtaining a WP, the following package of documents (depending on the type of hoisting
works) must be submitted to the Contractor’s OHFISE division:
‒ corresponding RLHD certificates and certificates of maintenance of general-purpose load-handling
devices (slings of appropriate load capacity and length used on HMM) in a serviceable condition;
‒ qualification documents of HMM operators and slingsmen;
‒ documents confirming that all employees involved in HMM operation have received on-the-job brief-
ings;
‒ approved and issued WEP/Complex WEP/PFC for the use of HMM;
‒ familiarization of all employees involved in the execution of works with the use of HMM with the ap-
proved WEP/Complex WEP/PFC against their signature;
‒ availability of a warrant from the Chief Power Engineer’s office (when working near power lines);
‒ slinging diagrams;
‒ up-to-date certificate of periodic control of HMM;
‒ HMM load capacity diagram.
WP shall be drawn up in accordance with the form specified in Appendix 1 with the check-lists
given in Appendix 9 attached.
6.9 Work Permit for Works in CES
CES works which require issue of a WP include works performed under the following conditions:

Working medium is considered unsafe if the oxygen level is less than 19.5% or more than 23.5%.
According to the regulations, the following levels of observed risks are unacceptable:
‒ concentration of combustible gas or its vapours exceeding the LEL by 10%;
‒ concentration of combustible airborne dust making it difficult to see at a distance of not more than one
and a half meters (1.5 m);
‒ concentration of a substance in the air that exceeds the permissible values specified in the material
safety data sheet for that substance.
WP shall be drawn up in accordance with the form specified in Appendix 1 with the check-lists
given in Appendix 10 attached. The check-lists for CES works shall be filled in on a daily basis.

Having listed the necessary safety measures, the Responsible Work Supervisor must specify the
personal protective equipment (PPE) that will be required during the works.
Prior to commencement of works, the WSA must be reviewed by the Contractor’s and Sub-con-
tractor’s OHFISE division and agreed upon by the management of the construction unit of the respective
site.
the danger at the Site (including installation of signs and fences), notify the WRP of related and/or con-
current works, and indicate these circumstances in the WP completion section.
Upon completion of works, the signed original work permit is kept on file with the Contractor’s
WP coordinator for at least 90 days. The register of work permits shall be kept for 1 year from the date of
the last entry.
8.3 Stoppage of Works
Works will be stopped if the following violations are detected:
‒ violations of OHFISE requirements and safety measures specified in WP;
‒ use of improper PPE, tools or equipment;
‒ absence of WRP in the work area;
‒ performance of works outside WP;
‒ performance of works without a work permit;
‒ performance of works without WP daily extension.
In the event of a hazard in the area of works under WP, the worker who first noticed the hazard
must notify the WRP and the OHS officer in the work area about the hazard. After that, the work must be
stopped, mechanisms and equipment de-energized, workers evacuated, explosive and flammable sub-
stances and materials removed from the hazardous zone.
If the works are interrupted for more than 4 hours, the WRP and the Sub-contractor’s OHS officer
in the work area must notify all work participants that the WP has been cancelled and a new WP must be
issued to continue the works. The new WP must be issued in compliance with the same requirements as
the previous one.
In the event of an emergency situation, all works under WP are stopped and WP within the area
affected by the ES are cancelled.
Elimination of emergency situations can be carried out without the issuance of WP under the di-
rect supervision of the responsible person. Such works must be organized in compliance with all safety
measures. After the elimination of the emergency situation, the works are continued as usual.
In the event of work stoppage, the employee who stopped the works must notify the RWS and the
head of the Contractor’s structural subdivision, and write down the reason for the work stoppage into the
form of the relevant WP.
8.4 Delegation of Duties
During his/her absence from the work site, WRP must delegate his/her duties to the DWRP, who
will replace him/her and will be fully responsible for the performance of the works under WP. WRP re-
sponsibility is resumed and the performance of duties is reassigned to him/her upon his/her return to the
work site.
In the event of WRP replacement during the validity of a particular WP, the DWPR taking up
his/her duties must read the WP and accompanying documentation, as well as sign the WP and WSA.
8.5 Training
In addition to having sufficient skills for the work under WP, employees must have certain qualifi-
cations, training and knowledge of the procedures and tasks to be performed under WP, as well as the
necessary training organized by the Contractor, such as risk analysis, safety lockout/tagout, safe perfor-
mance of hot works, CES works, handling chemicals, use of firefighting equipment, self-contained
breathing apparatus, scaffolding, HMM, operation of special equipment, etc., so that the employees know
the procedure for achieving maximum safety in the performance of works under WP.

Prior to signing a work permit, the person designated as the head of the Contractor’s structural
subdivision, RWS and WRP must be trained on the work permit system.
In order to develop and maintain the necessary skills in terms of compliance with the requirements
of this Procedure, the Contractor shall develop a training program for all persons involved in the process
of approval and performance of works under WP.
Within the work permit system training, the following issues are covered:
‒ principles of WP registration system operation;
‒ necessity for WP preparation;
‒ understanding of the content of WP and attached documentation;
‒ responsibility and competence of signatories (WRP, RWS) as related to the WP issuance system;
‒ hazard identification;
‒ efficient use of WSA.
8.6 Other information
The more information is attached to WP, the more hazards can be neutralized or their impact on
workers can be minimized, thereby making work safer. In this regard, it is necessary to attach layouts of
work areas with reference to specific objects of construction, as well as documents on the equipment used
Work area boundaries must be clearly marked (e.g. by a rigid fence). Works within the scope of a
particular WP cannot be carried out outside the boundaries specified in the permit.
When working in areas where there is a possibility of damage to hidden utilities, a “safe work
area” must be marked around the perimeter of the work site. The “safe work area” must be thoroughly in -
spected for hidden utilities. Any structures/utilities found must be clearly marked and discussed at a pre-
job briefing. A hidden utilities diagram is to be appended to the corresponding work permit.
When working with any chemical substances and materials, a safety data sheet for chemical sub-
stances and materials must be attached to the work permit.
In case works are performed using construction machinery and equipment, operation manuals and
safety certificates for such machinery and equipment must be provided; in the case of HMM, certificates
for RLHD and up-to-date certificates of periodic control are to be included.
Lack of necessary documents can be a reason for WP cancellation.
WP should be accompanied by check-lists for the actual type of works, WSA, as well as agreed
and approved WEP/Complex WEP/PFC.
In the event of an incident during works under WP, the original WP is kept with the materials of
the investigation.
In case there is not enough space in the WP form, additional sheets of the List of Work Performers
must be attached to the WP. Additional sheets must refer to the original WP number.
8.7 Works not Requiring a Work Permit
RWS may authorize certain works not affecting the safety of workers and adjacent work areas, as
well as not preventing the evacuation of workers, if it is agreed with the OHFISE Division of the Contrac-
tor.
‒ RWS shall make sure that risks relating to work performance are as low as practically possible.
‒ Such works may include the following:
‒ works within the current operation process;
‒ daily checks of measuring equipment;
‒ periodic maintenance (lubrication);

9. OUTAGE/INTERLOCK/ISOLATION CERTIFICATE
An outage/interlock/isolation certificate shall be attached to WP and contain the following infor-
mation;
list of equipment (points, cabinets, cells, lines, plugs, etc.) which will indicate the locations of out-
age/interlock/isolation required to perform works;
‒ coordination of points of outage/interlock/isolation;
‒ coordination and registration of the switch-ons/openings upon completion of works.
Note: an outage/interlock/isolation certificate must be prepared prior to approval of a correspond-
ing WP.
