1.0 Purpose:

The primary concern of TIIC management is to ensure that appropriate protection is


accorded to personnel from hazards arising in the conduct of critical work activities.

This procedure is written to serve as a guideline in properly managing the Work


Permitting System in TIIC.

The system is established to help control certain types of potentially hazardous work and
to establish communication among the personnel involved in carrying out potentially
hazardous jobs inside TIIC facilities

2.0 Scope:

This procedure will be applicable to all service contractors doing work inside any TIIC
facility and will cover not only general work activities but also hazardous works such as
but not limited to hot works, confined space entry, excavation, working at heights and
energized work.

5.1 PERFORMING PERSONNEL – is the person or CONTRACTORS involved in


carrying out the task, these persons may be:
 WELDERS
 SCAFFOLDERS
 HEAVY EQUIPMENT OPERATORS
 TECHNICIANS
 CARPENTERS
 ENGINEERS
 MASONRY WORKERS
 AND OTHER CONTRACTOR SKILLED PERSONNELS

These personnel shall ensure that the work is covered by a valid PTW procedure prior to
doing any work inside TIIC.

5.2 AREA MANAGERS/OWNERS – is the person(s) in charge of the work area and/or
project. He shall be authorized by the ISSUING AUTHORITY to supervise the
PERFORMING PERSONNEL to work safely in his area and must:
 Understand the hazards associated with the permitted work and must
review the PTW with the ISSUING AUTHORITY before allowing
contractors to start working.
 Not allow any unauthorized individual to perform or enter the permitted
work and area.
 Coordinate with the ISSUING AUTHORITY in conducting risk
assessments and help with identifying agreed controls.

5.3 ISSUING AUTHORITY (Safety Officer) – Shall be the responsible party for
ensuring that PTW system is implemented. He has the overall responsibility for
ensuring that the working area is rendered safe and the required controls are specified
and included in the PTW. The ISSUING AUTHORITY shall:
 Understand the potential hazards with the work covered by the PTW.
 Be competent in identifying the hazards and ensure that specific controls
are rendered.
 Have all PTW registered and constantly monitored (document filing,
formulate a PTW registry & on-site walkthrough inspections)

5.4 PTW HOLDER – is the person designated to be responsible in the application of


the PTW. He shall:

 Establish a working relationship with the ISSUING AUTHORITY with


regards to identification of conflicting/existing activities before applying
for a work permit.
 Maintain a PTW tracking consisting of filing and displaying issued
permits (active/suspended) at the working area.
 Engage/exert with the ISSUING AUTHORITY on the prescribed hazard
controls specified on the PTW procedures and ensure that risk rating is
brought to a minimum or ALARP.
 Maintains copies of filed PTWs during the duration of the project.
 Ensure that all workers are protected from the hazards and assert himself
as a safety advocate.

6.0 Guidelines and Procedures

During the project execution, the Safety Officer shall CONTRACTOR(S)


constantly check the work area if agreed safety arrangements
are maintained during the whole project duration. If any AREA MANAGER(S)
violations to permit provisions and agreed control measures
are found, the activity will be stopped until appropriate SAFETY OFFICER

The Work Permit will be valid only to the a.) work activity, b.)

document. Another permit will be applied should there be
changes to the information supplied in the original permit
application.

After finishing all of the work activities, the CONTRACTOR(S)


C CONTRACTOR shall turn over the project
ensuring that appropriate housekeeping is
conducted and that no unsafe conditions are
created after work completion.

The PTW HOLDER shall also endorse the


Work permit original PTW document to the SAFETY
Closure OFFICER for work closure.

SAFETY OFFICER
The jobsite is then inspected by the SAFETY
End OFFICER. Any concerns are immediately
brought to the attention of the CONTRACTOR
for immediate corrective action.

If no concerns are observed, the PTW is then


marked as closed and kept on file by EHSS for
at least 12 months.

Only after a PTW is closed can any barricade,


cordon or temporary board up be removed.
