AGREEMENT
This Agreement is made on this date 31 st May 2020 between M/s SAMPRE
NUTRTIONS LIMITED, plot no 133,Industrial estate, Medchal - 501401, Medchal -
Malkajgiri District, Telangana State having Place of business at Plot No: 127 & 128,
Medchal Industrial Estate, Medchal- Malkajgiri Dist, Telangana State-501401

AND

M/s. Sadhana Management Services represented by its proprietor Mr. K.VenkatRao

S/o Rama Rao, aged 57years residing at H.NO:5-3-432/3, RD NO:9 Venkat Rao nagar
colony Kukatpally, Hyderabad -500072 (Here in after referred to as the contractor).

Whereas the management is desirous in engaging outside party as contractor for

arranging and deputing sufficient certificate trained man power for boiler operation
and maintenance of 2.0 ton boiler & carry out the given assignment to them, and
where as the contractor offered to provide the necessary arrangement and service as
required by the management for which the management has agreed to utilize the
service of the contractor without any undue influence, coercion and fraud under the
terms and conditions as stated hereunder.

Terms and conditions

1. You will be paid monthly Rs 37,553 (Thirty Seven Thousand Five Hundred
and Fifty Three only) for maintenance of boiler and supply of manpower.
2. You are supposed to arrange manpower for Day Shift with reliever. (i.e.,
Nos.2 Boiler operators and Nos.2 Helpers.
3. You must obtain the required licenses under the various enactments like
the contract labour licenses, EFI, PF, PAN Card and professional tax etc.
4. You are supposed to inspect the boiler and carry out various checks and
adjustments.
5. The boiler operators must passes with minimum second class boiler
attendant certificate of competency.(Rule 21 & 42)of A.P Boiler Attendants
rules,1956. And boiler operators certificates (photo copy) must submit at
the time of including.
6. Each boiler operator is eligible to work for 8hrs duty incase or as per
requirement it may extent maximum of 16hrs if any person exceeds more
than 16hrs duty the same will be deducted from your bill.

7. You should ensure continuity of boiler operator including helpers at any
given point of time even in case reliever does not report for the duty.
8. At the time of boiler inspection your supervisor must be present till
completion of inspection
9. Before boiler inspection, boiler must be kept clean in good condition by
your helpers under your supervision. (in case of emergency welding tubes
etc. should carry out by you.)
10. The contractor shall ensure that the operating procedure is per equipment
design & manual, and all safety measures the operators are conversant and
are being followed without exception.
11. The contractor shall train the personnel on operation, efficiency, safety, &
usage of personal protective equipments.
12. The contract will be for a period of one year initially which can be renewed
for further period based on performance.
13. The contract may be terminated by giving 30days notice by either side.
14. Monthly payment will be made on 21st of next month.
15. All safety equipment will be provided by company.
16. If any operator or supervisor found without PPE during working hours as
per rules Rs.100/- will be deducted for first 3 incidents later person will be
terminated without initiation,

17. Any incidents (threat/intentional activities) which can effect food safety
need to be reported to sampre management.
18. Awareness Training should be provided to workers regarding food safety/
threat security topics.
19. Background verification should be done while recruitment of new joiners

In witness where of both the management and the contractor have here in set
their hands on this day 31st May 2020.

AGREEMENT
This agreement is made on this Date-19.03.19between M/s. SAMPENUTRITIONS
LTD here in after called the first party and FAST TECH ENGENEERING here in
after called second party as agreed to AC Maintenance of SAMPRE NUTRITIONS
LTD. It has become expedient enter into an agreement between the parties.

TERMS AND CONDITIONS

1. AC Mechanic should visit twice in a month and when required
2. The contractor shall ensure that the operating procedure is per equipment
design & manual, and all safety measures the operators are conversant and
are being followed without exception
3. . The contractor shall train the personnel on operation, efficiency, safety, &
usage of personal protective equipments
4. The contract will be for a period of one year initially which can be renewed for
further period based on performance
5. The contract may be terminated by giving 30days notice by either side.
6. Monthly payment will be made on 21st of next month.
7. All safety equipment will be provided by company.
8. The tenure of the agreement initially for the period of one year from 19.03.19
to 20.3.2020.
9. Any incidents (threat/intentional activities) which can effect food safety need to be
reported to sampre management.

10. Awareness Training should be provided to workers regarding food safety/ threat
security topics.

11. Background verification should be done while recruitment of new joinees
