Living with parents, only child from mother, have six siblings from father side.

History of High blood pressure from father side.

Educational History

Started Elementary in Cabungahan Elementary School at province of Masbate until grade III, transferred
in Sauyo Elementary School at Quezon City from grade IV until graduated, Starts Junior High School at
Sauyo High School until grade VIII and transferred to Rondina-Atendido National High School at the
province of Masbate until graduated with honors, finished Senior High School at St. Augustin School of
Nursing Quezon City.
IV. BEHAVIORAL OBSERVATION

Based on what I see of him or what I notice while interviewing him, he is happy to answer my questions,
especially since he transferred to different schools while he was still in Elementary and High School.
During the whole semester I noticed that he is a quite person. No energy to chat with other people in
the room. But he’s okay to talk to, and get along well.

marital relationship. He is also having difficulty with his work and depends on colleagues. He feels he has
specific ability and existence but tends to fear difficulty.He’s lack of motivation for his achievement in
life but still desires material things for his family as well as for himself. But Mr. Pastoril seems hes
