AFFIDAVIT OF NON-OPERATION

I, ____________________, of legal age, Filipino, married and a resident of

________________, Philippines, having been duly sworn according to law, hereby deposes
and states that:

1. I am the Accounting Office of ____________________________, a business entity

duly organized and existing under the laws of the Republic of the Philippines and
with office address at __________________________, Philippines;

2. On ________________________, the operation of the said business temporarily

stopped due to General Community Quarantine (GCQ) by the Government due to
the continued increase in the number of positive cases for COVID-19 and for the
best interest of the entire employees of _________________________;

3. Due to sudden temporary cessation of the said business, there was no remittance of
the Pag-ibig contributions of our employees for the month of _____________ as
______________ was not operational during such period;

4. Since the business temporarily stopped its operation, our employees were not
receiving compensation during the said period, hence we are not deducting our
employees monthly remittances for Pag-ibig during the said period;

5. I am executing this Affidavit to inform the office of Pag-ibig pertaining to non-

operation of _________________, and to attest to the truth of all the foregoing
statements and for whatever legal purpose this may serve under the premises.
