MANDATORY REQUIREMENTS
The applicant shall submit at least five (5) sets of the
following mandatory requirements:
1. Petition/Proposal for Declaration of an area as
Minahang Bayan duly notarized, properly signed
and executed by the petitioners.
2. Location Map of the proposed Minahang Bayan
area showing its geographic coordinates/meridional
block(s) and boundaries, using a National Mapping
and Resources Information Authority (NAMRIA)
topographic map in a scale of 1:50,000 or 1:10,000
duly prepared, signed and sealed by a deputized
Geodetic Engineer;
3. Area Status (MGB RO)
4. MGB RO Evaluation Report re:
4.1 Area Viability for Small-scale Mining
4.2 Allocation for Mineral Processing Zone (if
applicable)
If Petitioner is a registered organization:
5) Proof of registration with the SEC/DTI/CDA or
other appropriate government agency: Provided, that
the applicant shall be 100% Filipino. (e.g. Articles of
Incorporation and By-Laws, originally certified by
concerned agency)
5.1 Registration Fee: PhP 1,020.00 per
document
6) Sworn Board Resolution signed and executed by
the members of the board authorizing the President or
Chairman of the company/ applicant to execute
documents in behalf of the corporation;
6.1 Registration Fee: PhP 220.00 per document
