1) INTRODUCTION

a. Purpose

This specific procedure for the lifting is prepared in order to ensure that the
lifting job is done in a proper and safely manner. All aspects of works have been
studied and considered. As the lifting contractor, this procedure will be the
guideline during executing of any tasks and completion of the works.

b. Objective

This procedure will be a guideline to ensure the preparation and lifting


equipment’s or machineries has the right capability to assist and executeoverall
works. This method shall be followed once the approval is granted.
2) PREPARATION FOR MOBILIZATION

a. Site access for mobilization

After safety induction and documentation are completed the crane lifting crew need
to gain access to the lifting area. They have to survey, study and analyzed thecrane
and the transporter lifting position and movement to ensure the safe execution of
the lift.

A supervisor and the crane crew will do this survey on the site area to determine if
any obstructions exist along the path of the crane and transporter movement.

Besides that, the crane crew will also make the actual marking on site to place the
steel support plate for the crane. These marks will help the other project crews to
place the steel plates earlier and ensure the smooth operation.

The following equipment are required for the preparation of the mobilization
access and the crane path area.

b. Other preparations

i. Execution meeting

• To confirm duties of all personnel involved


• Review procedure & method statement
• Ensure signal given understood by all personnel
• Start lift when everything safe and ready
ii. Inspection

All equipment shall undergo OWNER’s Safety Inspection and must fully
comply with client’s requirement. Documents to be submitted for
inspection:

• Crane’s 25 Ton
• Operators driving license
• Insurance cover notes
• DOSH certificates
• PUSPAKOM certificates
• Crane load chart

iii. Rigging equipment

• All rigging equipment such as wire rope sling, shackle, webbing


sling must be approved and certified.
• Ensure lifting equipment is in good condition.
• Ensure lifting equipment rated is not down rated.
• Lifting equipment certificate to be produced prior to lift.
• Tag line shall be installed to control the load during the lift.

iv. Barricade and signage

• CONTRACTOR shall be double barricade the work area with


warning / dangerous tape and must display the signage with word
“LIFTING IN PROGRESS”.
v. Duties and responsibilities

• Contractor shall assign a lifting supervisor to supervise and


monitor the activities being carried out in safety manner.
• Supervisor shall ensure that lifting gears and lifting appliances are
in good condition and safe working load stamped.
• Supervisor ensures lifting gears are not down rated.
• Safety supervisor to conduct toolbox meeting to ensure all worker
understand the work sequence.
• Safety supervisor to check all workers have their correct and
sufficient required PPE.

JOB METHOD STATEMENT FOR SCRAP DISMANTLE

a) Setup crane 25 ton at designated area


b) After finish setup crane, check surrounding area to make sure all area safe and ready
to lifting activities.
c) Rigger will install lifting gear for material that will be lifted.
d) After get permission from lifting supervisor to proceeds the lifting, crane operator
will lift the material and place to designated location.
e) During lifting, signalman and rigger will make sure the lifting area is safe from hazard
and workers.
f) Above 2meter height work area, manpower/workers must at all time wear a safety
harness.
g) Life line must be install if necessary for safety purpose.
h) After all finish, rigging tackles will be dismantle by competent riggers.
i) All metal cut shall be of dimensions which are manageable to secure, load/lift/lower,
transport and which will minimize risk of injury of operatives.
j) Excavator c/w cutter and Oxy acetylene cutting will be applied at this stage
k) Trucks will be available to transport the material out from project site.
l) Properly housekeeping after finish all job.
