I acknowledge and agree with the commission payout scheme for the following rental sales order number

Rental Sales : Mattress Only Mattress + Bed Frame

Payment Date : 20th of each month

1st Month : 40% of the basic commission
2nd to 24th Month : 60% of basic commission divided equally for 23 months

I further agree:-
I. The above payout shall only be applicable for Rental Sales – Coway Mattress only
II. The full amount of the basic commission for the above Rental Sales – Coway Mattress is only
applicable for accounts a FULL ADVANCE PAYMENT for 2 years (24 months) has been made by
customer.
