AFFIDAVIT OF SELF-ADJUDICATION

I, MARY JOY CHAN, Filipino, single, of legal age and a resident of #123 Pinipig Street,
Quezon City, under oath, do hereby depose and so state that:

1. I am the sole surviving heir of ANNA MARIE CHAN who died intestate in Quezon City
last January 15, 2023 as evidenced by the Death Certificate issued by the Philippine
Statistics Authority and attached hereto as ANNEX A;
2. The decedent left behind a 750-sqm property located as 456 Macapuno St. Quezon
City with Transfer Certificate Title No. 78911;
3. The said property is more specifically described as follows:

TECHNICAL DESCRIPTION (copied from the TCT)

Bounded on the North by a private lot;

Bounded on the South by a mango tree farm;
Bounded on the East by a swimming pool; and
Bounded on the West by a poultry farm.

4. The decedent left no debts and monetary obligations upon his death;
5. The estate and inheritance taxes, as well as the real property taxes on the subject
land, have already been settled and paid on February 1, 2023 as evidenced by the
Bureau of Internal Revenue and attached hereto as ANNEX B;
6. This affidavit is being executed pursuant to the provisions of Section 1 Rule 74 of the
Rules of Court.
