AFFIDAVIT

I, AYRA CAMILLE GARCIA BORJA, Filipino, of legal age, single and residing at Blk 2
Lot 13 Camella Homes Silang, Cavite, Barangay Buho, Silang, Cavite after having been duly sworn to in
accordance with law do hereby depose and say that:

1. I am the owner and holder of a prepaid GLOBE SIM Card No. 09173148392 registered to
Gcash Account, AYRA CAMILLE GARCIA BORJA.

2. That currently, I am no longer using the aforesaid sim card as it has already been expired
last December 15, 2022.

3. That on January 4 2023, I received a money transfer from a client, however I cannot claim
the same since I am no longer receiving an OTP from the said simcard. Hence, in order to
collect the money transferred from my expired Gcash account (GLOBE SIM Card No.
09173148392), I respectfully request that my Gcash account from GLOBE SIM Card No.
09173148392 be changed to my new GLOBE SIM Card No. 09176312090;

4. I executed this affidavit to attest to the truth and veracity of the foregoing statements, to
apprise the authorities concerned particularly GLOBE Telecom.

law do hereby depose and say that:

5. I am the owner and holder of a prepaid GOMO SIM Card No. 09760864087 registered to
Gcash Account, MARIE ANGELICA TROPICALES.

6. That currently, I am no longer using the aforesaid sim card as it has already been expired
last October, 2022.

7. That on October 9 2022, I received a money transfer from a client, however I cannot claim
the same since I am no longer receiving an OTP from the said simcard. Hence, in order to
collect the money transferred from my expired Gcash account (GOMO SIM Card No.
09760864087), I respectfully request that my Gcash account from GOMO SIM Card No.
09760864087 be changed to my new GLOBE SIM Card No. 09455441606;

8. I executed this affidavit to attest to the truth and veracity of the foregoing statements, to
apprise the authorities concerned particularly GLOBE Telecom.

10. That currently, I am no longer using the aforesaid sim card as it has already been expired on
19th January 2023.

11. That on January 17th 2023 I accidentally transferred my money to my TNT SIM Card No.
09095819735, however I cannot claim the same since I am no longer receiving an OTP
from the said simcard. Hence, in order to collect the money transferred from my expired
Gcash account (TNT SIM Card No. 09095819735), I respectfully request that my Gcash
account from TNT SIM Card No. 09095819735 be changed to my new SMART SIM Card
No. 09993217543;
