RESEARCH INSTRUMENT

Dear Participants,

I am _______________ from University of Cebu, Graduate School. This research


pertains to the PARA-TEACHER COMMUNITY EXTENSION PROGRAM:
EXPERIENCES OF SELECTED MOTHER- BENEFICIARIES, I am going to give
you information and invite you to be part of this research. You do not have to decide
today whether or not you will participate in this research. Before you decide, you can talk
to anyone whom you feel comfortable with about this research.
