Lifestyle and Diet Plan Rubrics

Performance Task Description:


✓ Students will act as Nutritionists and create a lifestyle and diet plan for
two clients with different lifestyle and nutritional needs (Mr. Tan and
Sally).
✓ Students will present the personalized diet plan to the clients and
convince them to adopt the plan.
*Note that because face to face “presentation with the clients” will not happen as there
is not enough time for classroom presentation, it should be recorded instead, in a video
not exceeding 10 minutes.

Criteria and Basis of Scoring:

1 – does not meet expectations

2 – almost meets expectations

3 – meets expectations

4 – exceed beyond expectations

Understanding of the Subject Matter (40%)

1. All recommendations related to healthy diet and lifestyle are facts-based and evidently
researched.
2. Different factors that affect the health and overall well-being of a person are well researched,
clearly identified and considered in the creation of diet and lifestyle plan.
3. Knowledge learned about nutrients are evidently applied in the construction of the plan.
4. Nutritionists have a clear understanding of their clients’ background when it comes to their
current lifestyle and nutritional needs/status.

Structure/Organization of Plan (40%)

1. The lifestyle and diet plan are presented in an organized format which clients could easily digest
or understand. The details/elements included in the plan are comprehensible and can be easily
followed by the clients.
2. The lifestyle plan includes not just healthy but appropriate activities or practices that will fit well
the current health condition and lifestyle of the client.
3. The diet plan includes not just healthy but right choices of nutrients that will benefit the current
health status of the client.
4. The lifestyle and diet plan reflects clearly the goal of the client as well as the nutritionist.
5. Details like calorie intake, serving sizes, sleep time, healthy hobbies, exercises etc., that might be
included in the plan are correctly used and accurately presented.

Creativity/Impact of the Presentation (20%)

1. Nutritionists are doing well in explaining to the clients how lifestyle and diet affects the health of
a person.
2. Presentation of the plan appeals on the client’s emotion and correct judgement.
3. The use of multimedia tools enhances the overall appeal and the convincing power of the
presentation – clear audio, animation, pictures, text etc.
4. Presentation flows smoothly and not confusing in the part of the clients.
