1.

There will be 3 sets of the petition and the client has to sign all the papers in all three
sets.
2. The 1st set will include the affidavit and it shall be notarized.
3. The 2nd and 3rd set will only be of the petition which will not include the affidavit.
4. Clients sign at the end of every page.
5. Affidavit should be notarized.
6. Where I’ve mentioned (CS) the client has to sign there.
7. Where there is written thumb there the client has to put his left thumb impression.
