Dear Ma’am/Sir:

Good day!

This is to inform you that your son/daughter _______________________________ of Grade 11 section

________________ has shown delinquencies in his/her academic performance in General
Mathematics for this quarter. If no interventions will be made, there is a possibility that he/she will
have a failing mark on his/her first quarter grade.

In connection with this, I would like to ask your permission to allow your son/daughter to attend
remedial classes from October 17-21, 2022 every 1:00 to 2:00 in the afternoon in the designated
venue.

By signing this consent, you also acknowledge your responsibility as parent/guardian to ensure that
your student will comply the following:

1. Attend remaining classes of the semester without absenteeism/tardiness

2. Comply all requirements with satisfactory level
3. Attend remedial classes with no absence, and
4. Ensure that the student will pass the summative exam after the remedial classes.

Furthermore, by signing the consent form, if by any chance the student will not be able to comply any
of the terms stated above without valid reason, then the student will have a failing mark after the
evaluation of the outputs.

It is also my best interest to help your son/daughter. However, I cannot do this alone. So, I am asking
for your full cooperation for this matter. Thank you very much!
