BTVTED PRE-TEACHER’S EVALUATION SHEET IN EDUC .14

Purpose: Your evaluation is requested to guide the BUCIT Teaching Internship Course
Facilitator, in determining a fair academic rating of student teacher/s assigned to
you. Likewise, this evaluation is intended to make the student-teacher aware of
his/her strengths and weaknesses and of areas of improvement.
Timetable: Evaluation of student-teachers shall be done on the last week of the internship
period.
Procedure: The cooperating teachers/supervising instructor should accomplish the
evaluation form and rate the student-teacher as objectively as possible. After
rating, the cooperating teacher/supervising instructor should discuss his/her
evaluation with the student-teacher. The accomplished form duly signed by the
school head be forwarded to the Course Facilitator.
The pre-service teacher should be evaluated using the scale below:
Excellent: Superior: Very Satisfactory: Satisfactory:
1.0 = 99-100% 1.5 = 94% 2.0 = 89% 2.4 = 85%
1.1 = 98% 1.6 = 93% 2.1 = 88% 2.5 = 83%
1.2 = 97% 1.7 = 92% 2.2 = 87% 2.6 = 82-83%
1.3 = 96% 1.8 = 91% 2.3 = 86% 2.7 = 80-81%
1.4 = 95% 1.9 = 90% 2.8 = 78-79%
Average and Passing:
2.9 = 76-79%
3.0 = 75%
Interpretation of Grades:
The meaning of adjectival ratings and the corresponding ratings on the scale can be
interpreted as follows:
