    Dear Ma’am/Sir:
    This is to request a refund for the down payment I have made last November 03, 2017, for the
    second semester of the S.Y. 2017 – 2018, amounting three thousand pesos (P 3000).
    The reason for this withdrawal is the unavailability of the subject General Chemistry Calculation
    (Chem 113) for a semester at least which is a pre-requisite for higher chemistry subjects.
    I attached herewith copies of my ID and the original copy of OR for the reference.
