establish and operate an Off-Site Betting Station at the same

3. By virtue of the Off-Site Betting Station Agreement entered into

with LUCKY 8 STAR QUEST INC., with E-SABONG
OPERATIONS LICENSE NO. 21-002 issued by the PHILIPPINE
AMUSEMENT AND GAMING CORPORATION (PAGCOR), I
hereby acknowledge by responsibility as an agent of the said
company to collect all the bets placed in my betting station and
remit all the funds I received on the company’s behalf;

4. I do acknowledge and recognize that failure on my part to timely

remit the money I received from the said betting station, shall
give rise to the presumption that I have gravely abused the trust
and confidence bestowed upon me by LUCKY 8 STAR QUEST
INC. and that I have misuse the same for my own personal gain.

5. I am executing this Affidavit of Undertaking freely and voluntarily,

for the purpose of immortalizing my undertakings contained
herein, as well as for whatever legal purpose said Affidavit.
