1. What is the nature of your supervisory responsibility/ professional relationship with the
applicant?
The undersigned was a former DILG Field Officer of Local Government Unit
(LGU) of Bacolor, Pampanga and presently supervised the Municipal Planning and
Development Office of the same LGU. The undersigned regularly coordinates
various programs, projects, and activities to the concerned office where the
applicant is presently deployed.

2. Please indicate period of his/her employment in your organization.


The applicant is an employee of the LGU Bacolor, Pampanga for around three (3)
years. Due to the lack of vacant permanent positions in the LGU, Mr. Dizon could
not be given nor offer a regular position.
3. In your opinion, is he/she qualified to handle the responsibility inherent to the position
he/she is applying for?
Mr. Dizon is qualified and will be able to perform the duties and responsibilities for
the said position due to his exemplary work ethics.

5. Based from your observation and encounters with the applicant, what are his/her
strengths?
Greatest strength of Mr. Dizon is he can work under pressure as well as his capacity
for ease understanding and immediately perform the duties and deliverables
provided to him with minimal supervision. Similarly, his commitment to job is
commendable in which there were times he rendered overtime work and on holidays
if necessary.

6. What do you consider as his/her major contribution to your organization during his/her
employment?
The undersigned as a former DILG Officer closely works with various offices
including the Human Resource Management Office (HRMO) in which Mr. Dizon is
currently assigned. The partnership of the DILG Bacolor and LGU Bacolor,
Pampanga led to several recognitions and awards achieved by the latter such as the
prestigious Seal of Good Local Governance (SGLG) thru the hard work of all the
employees including the applicant.
7. What other relevant data of professional or personal nature on the applicant do you
consider important?
The applicant shows professionalism in his work and able to accomplished task and
submit reports prior to the given deadline. In addition, he is able to maintain a
respectable and decent working relationship with his colleagues as the applicant is
easy to deal with.

8. In what areas do you think he/she needs to improve?


At present, the undersigned cannot think of weakness as the applicant is able to
consistently deliver his task not just from his immediate supervisor but also from
other offices especially the request of the undersigned who came from a National
Government Agency (NGA). Definitely, there will be a room for improvement upon
hiring of the applicant once given with a more challenging duties and
responsibilities.

9. Would you re-hire him/her? If not, why?


The undersigned would definitely rehire Mr. Dizon and will offer a plantilla position
if there will be available vacant position in the future.
