Re: Mrs. Jasmine Thompson, 75 years old

Dear Nurse,

I am writing to refer Mrs. Jasmine Thompson, who was underwent total right shoulder replacement. She
is set to be discharged today and will be needing continuous monitoring and care.

On 11 July, Mrs. Thompson was diagnosed with Right shoulder osteoarthritis which prompted her
admission for the abovementioned operation. Post-operatively, laboratory test were done with stable
results while her wound was clean and dry. In addition, her pain has been managed, Panadeine Forte,
30/500mg 2 tablets every 6 hours as needed, and cold compress every for hour were given on her right
shoulder.

During confinement, Mrs. Thompson’s vitals were unremarkable and daily assistance were provided. Her
physiotherapist were including discharge instruction during discharge plan. Mrs. Thompson will benefit
to have grooming assistance and Cleaxane administration for DVT prophylaxis. Keep arm sling intact for
a month without heavy-lifting. Please schedule her physio and hydro. Staple removal will be done during
follow up at Orthopaedic nurse specialist.

It would be greatly appreciated if you could continue to assist her on daily activities like showering, and
administer Clexane, subcutaneously for 4 days. Please note that she needs to wear arm sling and strictly
no lifting for 4 weeks. Kindly remind her that she has physiotherapy twice a week and hydrotherapy
once a week. Also, staples on her wound needs to be removed after 10 days at Orthopaedic Joint
Replacement Outpatient Department. Lastly, if you have any concerns on her condition, you can contact
the Nurse specialist at the hospital from Monday to Friday.
