1. Describe the incident.

The incident is that a new employee on the workplace for the first day of his work had his
hand injured after getting boxes out of a moving machine that is stacked from the very risky
part to grab it, and since the co-worker were not continue working on the machine rather
than stopping it first so it can grab safely.

2. What was the hazardous energy encountered? Elaborate on your


answer.
The hazardous energy that is presented on the video is an example of mechanical energy
because all the hazards that are associated with the usage of machines should adopt a safe
work procedure and the application of appropriate safeguards.

As seen here in the image there is a moving machine and the last box are on its side of the
rubber, but since carelessness and lack of common sense the worker grabbed and
unfortunately stopped the rubber from moving with his hands although there are gloves
presented it really does not save his fingers from being crushed on the machine.

3. What were the lapses in the procedure? How can they be avoided?

• Messy workplace – Since it is common in factories to have machines it should be


considered first to always clean up the mess around, especially if the area
has a lot of machines present around the surroundings.
• Negligence – The worker working for the machine did not bother to turn it off first
before the new employee grabbed boxes around the area of a moving part of the
machine that had rubber which led to an accident.

• Lack of machine guards – It should have machine guards to the machine to avoid
any accident and signs also for the workers to be aware on what hazards they would
be having when it had been contact so that they could be said to be as warned before
it happened.
