MEMORANDUM OF UNDERSTANDING

WHEREAS, the DEPARTMENT OF EDUCATION herein represented by its official, RAUL ALDO O. PONTOY, School
Head of Tabango National High School with office address at Poblacion, Tabango, Leyte and LOCAL GOVERNMENT
UNIT OF POBLACION herein represented by its Barangay Captain, HON. TEODULFO A. MATIVO, JR. with its office
address at Poblacion, Tabango, Leyte, have come together to collaborate and to make an application as partners, of the
Department of Education Disaster Risk and Reduction Management (DRRM);

WHEREAS, the partners listed below have agreed to enter into a collaborative agreement that the Barangay Health
Emergency Response Team (BHERT) will provide an assistance in ensuring health and safety protocols is observe at
all times by the concern learners and employees in case of emergency during face-to-face classes implementation;

WHEREAS, the partners herein desire to enter into a Memorandum of Understanding setting forth the services to be
provided by the collaborative;

WHEREAS, the application prepared and approved by the collaborative through its partners is to be submitted to the
Office of the Department of Education and Local Government Unit of Poblacion on July 22, 2017;

WHEREAS, the Local Government Unit of barangay Poblacion has been agreed as the active partner in pursuing quality
education of the said school;

WHEREAS, this partnership has proposed a coordination plan of action in the Local Government Unit of Barangay
Poblacion through the office of the BDRRC;

Whereas, the Local Government Unit of Barangay Poblacion extensively put emphasis in helping the school to support
its vision and mission in this time of pandemic;

NOW, THEREFORE, it is hereby agreed that by the Department of Education and the Local Government Unit of
Barangay Poblacion, as partners, that they will respond to school calls when learners/employees would need health
care assistance during face-to-face classes implementation;
