accordance with law, hereby depose and state that:

1. I was the holder and bona fide owner of the Talk ‘N Text
sim card with corresponding number XXXXXXXX

2. On April 11, 2023, while I was in the comfort room about

to take a bath with my phone on my hand, it slipped and fell in a drum
of water. Consequently, the cellphone and the Talk ‘N Text sim card
therein is now beyond repair.

3. As a result of which, and despite diligent efforts to recover

the same, I am not able to access my GCASH account registered

4. I am executing this affidavit for whatever legal purpose

and to request that my GCASH account under my name and number
