AFFIDAVIT OF LOSS

I, ALYSSA MARIE D. PAGDANGANAN, of legal age, Filipino and a

residential address as Block 13, Lot 26, Centennial Town Homes, Brgy. San
Isidro, Cabuyao, Laguna, after having been duly sworn to in accordance with
law, hereby DEPOSE and STATE:

1. That I graduated from University of Perpetual Help System Laguna,

Inc., with Associate in Hotel and Restaurant Management, on year
2009, and B.S. Hotel and Restaurant Management, on year 2011;

2. That sometime on May 2020, my important belongings were burned

on a house fire which included my Diploma and Transcript of Records
for the aforementioned courses, thus, due to the fire it had been
destroyed and can no longer be recovered;

3. That I am executing this affidavit to attest the veracity of the foregoing

facts to report the said loss of Diplomas and Transcript of Records
and for whatever legal purpose this may duly serve;
