INCIDENT REPORT

Sir,

This is to report the Pre-damaged of rice with an area of (0.52 and 0.82 ha) of
Mr. Manuelito Prudente and Maximo Franquia residents of Brgy. San Juan, they
reported the incident on January 20, 2021 and the Agriculture office / personnel
Validated on the next day of January 21, 2021.

The location of the affected area is at Sitio Kalusan, Brgy. Balungay, Panukulan
Quezon, as per ocular inspection, an estimated of 70% of 0.52 has and 45% of 0.82
has of rice area, were pre-damaged due to deposited silt soil (banlik) from on going
roads improvement near proposed sanitary land fill cause by heavy rain, it can
damage area of rice, thru emission of iron toxicity that is harmful for rice
productivity coming from deposited silt soil.

In line with this situation, the farm owner/tenant asking for damage assistance to
the Local Government of Panukulan. This is considered a man made cause and it is
not qualified for insurance covered.

As per recommendation, we recommend for agencies involved in the

construction of SLF to do a Mitigation Plan for the precautionary measures of the
agricultural land areas.
