duly sworn to in accordance with law, depose and state:

1. That we are the biological parents of MS. FAITH ALLYN R. RARANG who was born on May 16, 1996 in San
Martin De Porres Charity Hospital, San Juan City ;

2. That Ms. Rarang, our daughter, is in an exclusive dating relationship, as defined under Republic Act No. 9262,
with Mr. Jesse A. Besana III ;

3. That under RA No. 9262, a “dating relationship” refers to a situation wherein the parties live as husband and wife
without the benefit of marriage or are romantically involved over time and on a continuing basis during the course
of the relationship;

4. That we are aware and knowledgeable of the romantic development of their current and existing exclusive dating
relationship since 2019;

5. That we are aware and knowledgeable of Ms. Rarang and Mr. Besana’s all and future plans, including that of
residing in Canada and making a life together there;

6. That, we are executing this Affidavit to attest and declare the truth of the foregoing facts and for whatever legal
purposes that this Affidavit may serve.

General Hospital, Taft Avenue, Manila;

2. That Mr. Besana, our son, is in an exclusive dating relationship, as defined under Republic Act No. 9262, with
Ms. Faith Allyn R. Rarang;

3. That under RA No. 9262, a “dating relationship” refers to a situation wherein the parties live as husband and wife
