I am writing to request your permission to conduct a research study titled "Study Anxiety
and Academic Performance of Senior High School Students at the Notre Dame of
Salaman College."

The purpose of this study is to investigate the relationship between study anxiety and
academic performance among senior high school students at Notre Dame of Salaman
College. The study will involve the administration of a questionnaire to a sample of
students, as well as the collection of academic performance data from school records.

I assure you that the data collected will be kept confidential and will only be used for
research purposes. The results of the study may be presented at academic conferences
or published in academic journals, but individual student information will not be
disclosed.

I am seeking your permission to conduct this study within the school premises and to
have access to student records necessary for the research. I also request your

LETTER FOR PARTICIPANTS

Greetings Participants,

I am writing to request your participation in a research study entitled "Study Anxiety and
Academic Performance of Senior High School Students at the Notre Dame of Salaman
College". The purpose of this study is to investigate the relationship between study
anxiety and academic performance among senior high school students.

Your participation in this study is voluntary. If you agree to participate, you will be asked
to complete a questionnaire that will take approximately 5-10 minutes of your time. The
questionnaire will include questions about your study habits, anxiety levels, and
academic performance.

Your participation in this study will be kept confidential, and your responses will not be
linked to your name or any identifying information. The data collected from this study will
be used for research purposes only and will not be shared with anyone outside the
research team.
