Greetings of Peace and Prosperity.

We, The Bachelor of Elementary Education of Illana Bay Integrated Computer
College Incorporated, would like to ask for your permission to allow us to conduct a
survey among your teachers in your school, the Parang Central Elementary School.
This is in view of our study entitled “FACTORS AFFECTING THE EFFECTIVENESS
OF TEACHING PERFORMANCE OF ELEMENTARY TEACHERS IN PARANG
NORTH DISTRICT”. We are conducting a survey among the elementary teachers of
Parang North District.
This study aims to determine the factors that affect the effectiveness of
teaching performance of elementary teachers.
Specifically:
1. What is the demographic profile of the respondents in terms of:
a.) Age;
b.) Sex;
c.) Marital Status;
d.) Highest Educational Attainment;
e.) Length of Service; and,
f.) Teaching Position?
2. What are the Factors affecting the effectiveness of teaching performance of
elementary teachers in terms of:
a.) Age;
b.) Marital Status;
c.) Highest Educational Attainment;
d.) Experience; and,
e.) Monthly Salary?

The survey will be conducted as part of our requirements in our Subject THESIS, We
believe that the opinions and perspectives of the Teachers at Parang Central
Elementary School will greatly contribute to a comprehensive understanding of our
study. The information obtained from the survey will be used solely for research
purposes and will be treated with strict confidentiality.

To ensure the success of this research, we kindly request your permission to access the
school premises and seek the cooperation of the teachers. We will be responsible for
managing the entire survey process and ensuring that it adheres to ethical guidelines
and respects the privacy of the participants.

We will greatly appreciate your support in granting permission for this research
