ON-THE-JOB TRAINING AGREEMENT

JOHN B. LACSON FOUNDATION MARITIME UNIVERSITY-MOLO, INC.

hereby agree to accept

____THESSALONICA MARIE DE LARA___________

to participate in the Local Internship Program (Practicum 1- Passenger Ship Familiarization and Local
Internship) of the above university and to undergo an On-the-Job training at the above-mentioned
establishment under the following conditions:

1. The training shall be in any of the following for 150 hours:

a. Food and Beverage Service

(like setting up tables, banquet set-up, order-taking, waitering, bussing)
b. Assist in Food Preparation
(Such as receiving food deliveries & inventory, pre-preparation of ingredients)

2. Neither the UNIVERSITY nor the COMPANY shall be under any obligation to hire or secure a job
for the student-trainee after his/her training period;

3. The student-trainee shall not be made to work overtime (unless with written consent of the
parent/guardian and duly noted by the Practicum Coordinator), nor to take over the position of
any employee of the COMPANY, nor be compensated either as a substitute or a regular employee;

4. The UNIVERSITY and the COMPANY reserve the right to terminate the training of the student-
trainee for reasons found to be justifiable; and

5. The parents or guardian of the student-trainee shall be responsible for the personal needs and
conduct of their son/daughter/ward during the training period.

6. The UNIVERSITY shall hold the COMPANY harmless and free from liability for any and all
claims arising from or by any reason of the deliberate act or negligence of the student. Neither
shall liability against the UNIVERSITY and the COMPANY, its officers, employees, agents, and
representatives for the death of, nor bodily injuries and sickness sustained by any student in
consequences of accident or deliberate act or negligence of the third parties or strangers.
