Component I. Form
Technical Notes. The C/M will have to present prescribed or alternative/substitute forms and
documents to the CDP-TAC. The table below indicates the documents/forms that the C/M
should present. In cases wherein the required document/form has not been submitted, the
CDP-TAC may request the C/MLGUs to submit acceptable alternative documents.
The role of the DILG in the assessment of this Component is deemed critical given their
familiarity with the prescribed forms and documents for CDP formulation.
A. Forms
1. Harmonized Work Plan Harmonized work plan (Form 1b) should include activities
for CDP and/or CLUP formulation, consisting of major
activities as follows:
- Team Mobilization

3. Local Development Check for the accomplished LDI matrix (Form 1d) that
Indicator System (LDIS) should contain indicators for the five development sectors,
/ Rationalized Planning broken down into barangays, districts or any appropriate

(RaPIDS) data to be used is not based on a historical trend, the data
must not be older than 10 years prior to year 1 of CDP

4. Structured List of Structured list of PPAs (Form 2a) should contain the long

6. Ranked List of PPAs Check for accomplished Form 3a that should contain

Overall Interpretation
Guided by the Technical Notes for the Assessment, the CDP TAC shall evaluate the C/MLGU
compliance in terms of the following qualitative rating. The summative rating below shall be
indicated at the end of each section. For partial compliance, the CDP TAC shall indicate the
missing form, process, or content in the summary. For non-compliance, there is no need to
indicate such details.
Each interpretation shall be followed by a response or feedback from the C/MLGU. This
section shall contain all the comments, clarifications, explanations, information and/or any
additional responses from the C/MLGU on the findings made by the CDP – TAC.
