THIS IS TO REPORT THAT Vessel Holds Nos. 1, 2, 3, 4 and 5 were inspected by the undersigned
jointly with the vessel's Chief / Cargo Officer and COTECNA CARGO SURVEYOR, while said vessel
lay afloat and safely secured at CANAWAYON, HOMONHON IS., GUIUAN, EASTERN SAMAR, PHILS.,
prior to loading a cargo designated and said to be NICKEL ORE in (√ ) bulk ( ) bags

By careful visual inspection where accessible and free for direct examination, said hold(s) were found to be
apparently free from loose rust and sales, residue of previos cargo, free water and other foreign objects.
( ) The bilges were opened by deckhands, subsequently inspected and were found to be with nil or with
( √ ) The bilges were not opened by deckhands, hence, not inspected due to, NO VISIBLE SIGNS OF
OVERFLOWN WATER ON TOP OF BILGE'S COVER.
