I do hereby declare that I have PF/ESI’s of all the manpower deployed on aforementioned sites. We are
committed to adhere all the safety norms specified by SolarEra and proposed off-takers in whose premise’s M/s
_________________________________________would be performing PV module cleaning activity.

I confirm that each member of staff is aware of his/her responsibilities and are competent to perform module
cleaning activity. I also confirm that, we would be using below listed Personal Protective Equipment’s based on
the type of structure where our staff would be performing cleaning activity:

Limited shall not be held liable or responsible for any act of negligence, incident, injuries arising on workplace.

I declare that above mentioned statements are best to my belief and knowledge and will adhere to all the safety
norms and all the efforts will be taken to restrict the site to zero incident. If any violation committed by M/s
________________________________________employees/workers, then M/s SolarEra Green Renewables Pvt
Ltd is authorized to impose penalty on individual/firm.
