Most respectfully, I have been suffering from sore eyes for the last
weeks and the doctor diagnosed an infection in my eyes. The doctor has
therefore advised me to limit my exposure to any radiation from technology
and advised me to visit a week after to do a follow up check up.

I formally request a leave of absence to take care for my eye

infection and attend to personal matters, particularly school and family
matter. I humbly ask that you will be able to grant my request. Please let
me know if you require any additional information or have any questions
and rest assured that I'll make sure to return to work.
